摘要
目的评价高频振荡通气(HFOV)治疗早产儿呼吸窘迫综合征(RDS)的疗效。方法选取本院2008年1-9月收治的RDS早产儿70例。根据胎龄、体质量、年龄、病情等随机分为常频机械通气治疗组(CMV组,n=37)和HFOV组(n=33),分别给予CMV和HFOV治疗,比较CMV组和HFOV组患儿治疗前后血气分析指标的变化。结果HFOV组早产儿吸入氧体积分数(FiO2)、pa(CO2)在HFOV治疗6h后,与开始治疗时比较均显著下降(Pa<0.01),CMV治疗组早产儿FiO2、pa(CO2)则在开始机械辅助通气12h后,与开始治疗时比较有统计学差异(Pa<0.01),但下降幅度明显低于HFOV治疗组(Pa<0.01)。氧合指数(OI)在HFOV治疗12h后,与开始治疗时比较显著下降(P<0.01),明显低于CMV治疗组(P<0.01)。HFOV治疗组pa(O2)在HFOV治疗12h较开始治疗时明显升高(P<0.01),并显著高于CMV治疗组(P<0.01)。CMV治疗组发生气漏并发症高于HFOV治疗组(P<0.05)。结论HFOV与CMV比较,前者能更好地改善早产儿RDS的肺氧合功能,减少气漏等并发症。
Objective To evaluate the effect of the high frequency oscillatory ventilation (HFOV) on the treatment of premature neonates with respiratory distress syndrome (RDS). Methods Seventy premature infants with RDS were randomly grouped as HFOV group ( n = 33 ) and conventional mandatory ventilation group ( CMV group, n = 37 ) , based on their fetal age, weight, age, and their clinical condition from Jan. to Sep. in 2009. The blood gas analysis was detected and compared between the 2 groups. Results In HFOV group, the inhaled oxygen concentration,pa ( CO2 ) decreased after treatment for 6 h, which were significantly lower than those at the beginning of the therapy ( Pa 〈 0. 01 ) ;While in CMV group,they decreased after 12 h treatment and the decreased extent was lower than those in HFOV group(Pa 〈0. 01 ). For the oxygen index (OI) ,it decreased after 12 h treatment in HFOV group(P 〈0. 01 ) ,which was significantly lower than that of CMV group ( P 〈 0. 01 ). For the Pa ( O2 ), it increased after 12 h treatment in HFOV group( P 〈 0.01 ), which was significantly higher than that of CMV group ( P 〈 0. 01 ). The incidence of air leak in the CMV group was higher than HFOV group ( P 〈 0.05 ). Conclusion In the treatment of premature neonates with RDS, HFOV has a better pulmonary oxygenation and lower air leak than the CMV.
