彩超引导下肉毒毒素注射治疗脑卒中后上肢肌肉痉挛的疗效观察 被引量:3

摘要 目的评价超声引导下肉毒毒素注射治疗脑卒中后肌肉痉挛的效果。方法选择因脑卒中致上肢痉挛患者15例,在彩超引导下,按照断层解剖作为指南,将肉毒毒素准确注入到肱二头肌、肱肌、指深屈肌、指浅屈肌、尺侧腕屈肌、拇长肌、掌长肌、旋前圆肌、旋前方肌。注射后继续进行康复训练。注射前、注射后2周、注射后4周采用改良Ashworth(MAS)、Fugl-Meyer上肢功能评分进行相关评价。结果注射组在注射后2周、4周,肌肉痉挛的改善以及上肢运动功能评分均优于注射前。结论在彩超引导下肉毒毒素注射定位技术治疗脑卒中后肌痉挛,定位准确、效果明显。 Objective To evaluate outcome of injection of botulinum toxin A to treat post stroke muscle spasm under color doppler ultrasound guide. Methods 15 stroke patients following spasm in upper limb were chosen. Under the guidance of ultrasound, botulinum toxin A was accurately injected in biceps, brachialis, deep flexor muscle of fingers, superficial flexor muscle of fingers, musculus flexor carpi ulnarista, thumb muscle, palmaris longus muscle, musculus pronator teresta, and quadrate pronator muscle. All patients went on rehabilitation training after injection. Upper limb function was evaluated by using modified Ashworth(MAS) and Fugl-Meyer scores before injenction, two weeks and four weeks after injection. Results Recovery of muscle spasm and scores of the upper limb motor function were better than that before injection. Conclusion Under color doppler ultrasound guidance, injectionof the botulinum toxin A can treat post-stroke muscle spasm with accurate location.
