摘要
目的:通过对第一台手术按时开台时间、手术间的周转时间、手术时间、手术延时的情况进行调查与分析,找出影响手术间使用效率的原因与对策,提高手术间使用率。方法:召开围术期领导团队主持的手术团队工作讨论会,针对要解决的问题找出各种影响因素,制作影响手术室工作效率的"人、机、物、法、环"因素分析鱼骨图,找出提高手术室工作效率的对策,制定手术室效率计划。结果:计划实施后6个月,因手术、麻醉医生或手术间的准备不充分而造成的手术延迟现象减少了9%(P<0.01,χ2=5.38),第一台手术在预定时间的15min内按时开台比例提高了14%(P<0.05,χ2=3.36),手术周转时间可缩短12min(P<0.05,t=1.25)。病人的满意度和手术医生、手术室护士对工作的满意度提高了,病人得到更优质、更安全的护理,保证了手术室的护理安全与质量。结论:解决问题要有策略,执行好的围术期效率计划,可以提高病人和手术医生的满意度,也可以提高手术室员工的满意度以及病人的安全性。进一步提高了手术室工作效率,降低医院成本。
Objective: To find out the causes and strategy which affected the utilization rate of operating room by analyzing the on-schedule-opening rate of first operation, turnover time between operations, operating time and the status of delayed operation. Method: A brain storm meeting was organized by the leader of perioperative team in order to identify the problems affecting the utilization rate of operating room. All factors including human, machines, materials, pathways and environment were analyzed by using fish-bone diagram. Then the authors established some coping strategies and action plans to solve these problems. Results: After implementing the action plans for 6 months, the phenomenon of delayed operation caused by insufficient preparation of surgeons, anaesthetist and operating room decreased 9% (P 〈 0.01, x^2 =5.38). The on-schedule-opening rate (less than 15 minutes with the schedule) of first operation increased 14% (P〈0.05, x^2 =3.36). The turnover time between operations shortened 12 minutes (P 〈0.05, t = t.25). Patient satisfaction, job sarisfaction of doctors and nurses also improved. The quality and safety of nursing care in operating room were ensured. Conclusions: New strategies and action plans could improve the satisfaction of patients, doctors and medical staff of operating room. Patients' safety was ensured. Further more, it would reduce costs of hospital by increasing the utilization rate of operating room.
