摘要
目的:研究胰岛素样生长因子-Ⅰ(insu-lin-like growth factor-Ⅰ,IGF-Ⅰ)、胰岛素样生长因子-Ⅰ受体(insuline-like growth factor-Ⅰreceptor,IGF-ⅠR)和胰岛素样生长因子结合蛋白-2(insu-lin-like growth factor binding protein-2,IGFBP-2)在乳腺癌组织中的表达及其与乳腺癌临床病理学特征的关系及其在乳腺癌发病过程中的作用机制。方法:免疫组织化学SP法检测42例乳腺癌患者肿瘤组织和42例癌旁3cm外正常乳腺组织中IGF-Ⅰ、IGF-ⅠR和IGFBP-2的表达水平。结果:IGF-Ⅰ、IGF-ⅠR和IGFBP-2在乳腺癌组织中的表达率显著高于在正常乳腺组织中的表达,P<0.05。IGF-Ⅰ、IGF-ⅠR和IGFBP-2原发灶≤2cm的乳腺癌中的表达均低于原发灶>2cm的肿瘤,P<0.05;在有淋巴结转移乳腺癌中的表达显著高于无淋巴结转移乳腺癌,P<0.05。IGF-Ⅰ、IGF-ⅠR和IGFBP-2在乳腺癌组织中的表达相互具有明显正相关性,P<0.01。结论:IGF-Ⅰ、IGF-ⅠR和IGFBP-2的过度表达在乳腺癌原发灶的发展和侵袭转移可能起重要作用,IGFBP-2水平升高可能增强IGF-Ⅰ与IGF-ⅠR的结合产生活性促进肿瘤细胞的增殖从而与乳腺癌发生的生物学行为和浸润转移有关。
OBJECTIVE: To study the expressions of IGF-Ⅰ, IGF-ⅠR and IGFBP-2 in breast cancer tissues and the correlation among them and with the clinical pathological characteristics, and explore the mechanisms of development of breast cancer. METHODS: The expressions of IGF-Ⅰ, IGF-ⅠR and IGFBP-2 in each of the 42 cases of breast carcinomas as well as normal tissues 3 cm off the tumor region were determined by the immunohistochemical technique (SP method). RESULTS: The positive expression rate of IGFBP2 in breast carcinomas was significantly higher than that in normal breast tissues, P〈0.05. When breast cancers with primary sized 42 cm compared with that of 〉2 cm, the positive percentages of IGF-Ⅰ, IGF-ⅠR and IGFBP-2 in the former were significantly lower than those in the latter, P〈0.05. In addition, the positive percentages of them in breast cancers with metastasis in lymph node were higher than those of absence of metastasis in lymph node, P〈0.05. The expressions of IGF-Ⅰ, IGF-ⅠR and IGFBP 2 in breast tumor tissues showed a significant positive correlation (P 〈 0. 01 ). CONCLUSIONS: The excessive expressions of IGF-Ⅰ, IGF-ⅠR and IGFBP-2 may be the vital role in primary lesion development, invasion and me tastasis of breast cancer. The increased IGFBP-2 level can import IGF-Ⅰ into IGF-ⅠR to promote tumor cell proliferation, which sug gests that the three factors may cause biological actions and infiltra tion and metastasis of breast cancer.
