局麻患者不同剂量咪达唑仑复合芬太尼镇静效果的比较 被引量:6

摘要 目的探讨比较不同剂量咪达唑仑复合芬太尼对局麻患者的辅助镇静效果,寻找其适宜剂量,为临床麻醉提供参考。方法局麻患者500例。ASAⅠ或Ⅱ级。年龄18~60岁。局麻种类:硬膜外阻滞、蛛网膜下腔阻滞、脊椎-硬膜外联合阻滞、神经阻滞。本研究采用大样本、前瞻性、多中心、随机、双盲的试验,患者随机分为5组(n=100),Ⅰ组:氟哌利多0.5mg/kg复合哌替啶1.0mg/kg;Ⅱ组:咪达唑仑0.4mg/kg复合芬太尼0.75μg/kg;Ⅲ组:咪达唑仑0.04mg/kg复合芬太尼1.0μg/kg;Ⅳ组:咪达唑仑0.06mg/kg复合芬太尼0.75μg/kg;Ⅴ组:咪达唑仑0.06mg/kg复合芬太尼1.0μg/kg。结果咪达唑仑0.04mg/kg复合芬太尼1.0μg/kg或咪达唑仑0.06mg/kg复合芬太尼0.75μg/kg用于成年局麻患者辅助镇静效果好,明显优于氟哌利多复合哌替啶这一传统组合。 Objective To compare the sedative effect of different doses Midazolam Combined with Fentanyl in local anes- thesia in order to explore proper doses for clinic. Methods 500 cases of such patients (grade Ⅰ or Ⅱ of ASA, age 18 -60, including epidural block, subarachnoid block, vertebro - Epidural combined block and nerve block) were observated. This study is large sample, prosective, multicentre, random and double - blind experiment. The patients were divided into 5 groups randomly( n = 100). Group I : Droperidol 0.5 rng/kg combined with meperidine 1.0 mg/ kg, Group 1/: Midazolam 0.4 mg/kg combined with Fentanyl 0.75 μg/kg, Group Ⅲ : Midazolam 0.04 mg/kg com- bined with Fentanyl 1.0 gg/kg , Group Ⅳ : Midazolam O. 06 mg/kg combined with Fentanyl 0.75 μg/kg, Group Ⅴ : Midazolam 0.06 mg/kg combined with Fentanyl 1.0 μg/kg. Results It is obviously surpass the routine combination of Droperidol and meperidine that Midazolam 0.04 mg/kg combined with Fentanyl 1.0 μg/kg or Midazolam 0.06 mg/kg combined with Fentanyl 0.75μg/kg in adult local anesthesia cases.
