咪达唑仑复合丙泊酚、氯胺酮静脉麻醉用于小儿腹腔镜手术的应用观察 被引量:4

摘要 目的观察咪达唑仑复合丙泊酚、氯胺酮静脉麻醉在小儿腹腔镜麻醉中应用的可行性。方法选取108例行小儿腹腔镜手术患儿随机分为M组和D组,其中M组用咪哒唑仑复合丙泊酚、氯胺酮静脉全麻,D组用地西泮复合丙泊酚、氯胺酮静脉全麻,比较两组麻醉起效时间、手术时间、麻醉药物用量、术后苏醒时间、术中Ramesay镇静评分、患儿躁动发生率及术前、术中、术后血氧饱和度和心率变化。结果两组患儿均在静脉复合非气管插管全麻下完成手术。M组术后苏醒时间比D组明显缩短,差异有统计学意义(P〈O.叭),M组术中丙泊酚、氯胺酮的用量少于D组,差异有统计学意义(P〈0.05)。两组患者手术过程中心率和血氧饱和度均无明显变化(P〉0.05),两组术前、术中、术后的心率及血氧饱和度分别比较,差异亦无统计学意义(P〉0.05)。结论咪达唑仑、丙泊酚复合氯胺酮麻醉具有起效快,麻醉药用量少,对呼吸影响小、术后苏醒快等优点,是一项安全,有效可行的麻醉方法。 Objective To observe the compound midazolam propofl, ketamine intravenous anesthesia in pediatric laparoscopic feasibility ofapplication of anesthesia. Methods 108 children with routine pediatric laparoscopic surgery were randomly divided into M group and D group, M group compound midazolam propo- fol, ketamine intravenous anesthesia, D Group space diazepam compound propofol, ketamine intravenous anes- thesia, compared two groups of onset time of anesthes pmpofolia, operative time, Ramesay intraoperative seda- tion score,the incidence of children with restlessness and preoperative,intraoperative and postoperative Oxy- gen Saturation with Pulse Oximetry (spo2) and heart-rate changes. Results The two groups of children in the vein composite non-tracheal intubation under general anesthesia to complete surgery. M group postoperative recovery time was significantly shorter than the D group (P 〈 0.01 ), M group intraoperative propofol, ket- amine usage is less than D group( P 〈 0.05 ) o There are no visible variance in heart rate and spo2 during the operation between two groups ( P 〉 0.05 ). Compareing respectively the preoperative, intraoperative, postoper- ative heart rate and spo2, there are no statistical significance( P 〉 0.05 ). Conclusion Midazolam, propofol combined with ketamine anesthesia with effecct quickly, with less anesthetic, on respiratory effects of small, fast after waking up, ect, is a safe, effective and feasible method of anesthesia.
