摘要
目的:通过临床观察,探讨调免Ⅰ号联合熊去氧胆酸胶囊(优思弗)治疗自身免疫性肝炎(AIH)的有效性。方法:采用随机单盲的试验方案,观察调免Ⅰ号联合优思弗治疗45例AIH(肝肾不足兼有血瘀证候)患者的临床疗效,并以15例强的松龙联合硫唑嘌呤及优思弗的西医标准治疗方案为对照,治疗6个月、12个月后,观察患者症状体征、肝功能、γ-球蛋白及免疫球蛋白IgG的改善情况。结果:治疗6个月及12个月后,两组患者各项症状体征均有不同程度减轻,且治疗组在改善患者乏力、口干、月经失调、舌红方面优于对照组(P<0.05或P<0.01);治疗6个月及12个月后,两组患者肝功能(ALT、AST、TBil、ALP、GGT)、γ-球蛋白及免疫球蛋白IgG指标较治疗前显著改善(P<0.01),但两组之间差异无显著性意义(P>0.05);治疗6月后,治疗组临床总有效率为75.6%,与对照组73.3%相仿(P>0.05),治疗12个月后,两组总有效率分别达到77.8%和80.0%,差异亦无显著性意义(P>0.05)。结论:调免Ⅰ号联合优思弗是治疗自身免疫性肝炎(肝肾不足兼有血瘀证)患者的有效方案,其综合疗效与西医标准治疗方案相当,且具有更好改善患者症状体征、避免激素副作用的优势。
Objective： To study Tiaomian Ⅰ Combining ursodeoxycholic acid＇s effect of treating autoimmune hepatitis （AIH） through the clinic observation. Methods： Using single-blinded method, 60 cases diagnosed as AIH were randomly divided into 2 groups, 45 patients were treated with Tiaomian Ⅰ combining ursodeoxycholic acid for 12 months, while the controlled received the treatment of prednisolone （Pred） and azathioprine （AZA） . The patients＇ symptoms and signs, hver function, γ-globulin as well as serum IgG were observed before and after treatment. Results： After 6 or 12 months＇ treatment observation group could effectively improve the patients＇ symptoms and signs, and indicated the same effects to liver function, γ- globulin and IgG. Comparing with the controlled combining treatment has better impact on improving patients＇ debilitation, dry mouth, menstrual disorder and red tongue （P 〈 0. 05 or P 〈 0. 01 ）, but the other results had no significant differences （P 〉 0. 05） . The 6 month total effective rate of observation group was 75.6%, which had no significant difference with the controlled 73.3% （P〉0. 05） . After 12 month, the results reach 77. 8% and 80. 0%. They also had the similar effect （P 〉0. 05） . Conclusion： Tiaomian Ⅰ combining ursodeoxycholic acid is a kind of effective medicine to treat the AIH patients with in.sufflciency of liver and kidney, and blood stasis. It has the comparable effect with the standard treatment of Pred combining AZA, and hold an advantage of bettering complains and physical signs, and avoiding Pred＇s side effects.
