摘要
目的探讨睡眠呼吸暂停低通气指数与夜间血氧饱和度的相关关系。方法选用患有打鼾并伴有呼吸暂停的163例患者,行整夜7小时多导睡眠监测。分析其睡眠结构,计算出总睡眠时间、睡眠呼吸暂停低通气指数、夜间最低血氧饱和度及氧饱和度低于90%、85%、80%所累计时间占总睡眠时间的百分比。分析睡眠呼吸暂停低通气指数与夜间血氧饱和度的相关关系。结果两变量相关分析发现,睡眠呼吸暂停低通气指数与夜间最低血氧饱和度呈负相关(r为-0.614),睡眠呼吸暂停低通气指数与氧饱和度低于90%、85%、80%所累计时间占总睡眠时间的百分比呈正相关(r分别为0.434、0.519、0.503);经计算得出回归方程:LSaO2=87.92-0.372×AHI(R2=0.377,P<0.001),睡眠呼吸暂停低通气指数每增加10次/h,夜间最低血氧饱和度值下降3.72%。结论夜间血氧饱和度下降的程度和氧饱和度低于85%所累计时间占总睡眠时间的百分比可作为代表低氧血症严重程度的指标,还可以通过睡眠呼吸暂停低通气指数来预测推断最低血氧饱和度,通过最低血氧饱和度了解患者血氧程度,对指导临床诊断及其治疗具有一定意义。
Objective To investigate the relationship between the sleep apnea hyponea index and nighttime oxygen saturation. Methods 163 patients suffering from snoring and sleep apnea were undergone 7 hours polysomnography. To analyze the sleep structure and figure out total sleep time , sleep apnea hyponea index , lowest oxygen saturation at the time that occupied , the percentage of the total recorded time when the oxygen staturation below 90% ,85% and 80%. Moreover the correlativity of the sleep apnea hyponea index and nighttime oxygen saturation was analyzed. Results The sleep apnea hyponea index correlated negatively with the Lowest of Oxygen Saturation(r = -0.614), sleep apnea hyponea index was positively correlated with the percentage of the total recorded time spent below 90% ,85% and 80% oxygen staturation( r = 0. 434,0. 519,0.503 ) ;The regression equation was formulated to show the relationship between AHI and LSaO2 : LSaO2 = 87.92 - 0. 372 × AHI ( R2 = 0. 377, P 〈 0.001 ),sleep apnea hyponea index increased in 10 times/h, lowest of oxygen saturation valuation was dropped down 3.72%. Conclusion The degree of the nighttime oxygen saturation descending and the percentage of the total recorded time spent below 85 % oxygen staturation can represent the index of the severe hypoxemia, and get thought sleep apnea hyponea index to infer the lowest of oxygen saturation, wherefore, understanding the level of oxygen saturation. It is shown a significance for clinical diagnosis and therapy of the sleep apnoea hyponea syndrome.
