摘要
目的:探讨超敏C反应蛋白(hsCRP)与糖尿病及其肾脏并发症的关系。方法:根据尿白蛋白排泄率(UAER)将106例Ⅱ型糖尿病(T2DM)分为正常蛋白尿组(NA组,UAER<20ug/min>39例;微量蛋白尿组(MA组,UAER 20-200ug/min)35例;临床蛋白尿组(CP组,UAER>200ug/min)32例,采用免疫透射比浊法检测106例T2DM和33例对照组血清hsCRP含量。结果:T2DM组血清hsCRP为3.37±2.83mg/l,明显高于对照组1.25±0.68mg/(P<0.01);NA组、MA组和CP组hsCRP分别为2.12±1.87mg/l、2.92±1.55 mg/l和5.38±3.72mg/l,均明显高于NC组(P<0.01);NA组与MA组和CP组比较,差异亦有显著性(P<0.01);MA组与CP组比较,也存在显著差异(P<0.01);T2DM组血清hsCRP与UAER呈正相关关系,r=0.66(P<0.01)。结论:T2DM血清hsCRP水平明显升高,且与尿白蛋白排泄率正相关,血清hsCRP能够反映T2DM肾脏损伤程度。
[Objective]To study the relationship between high sensitive C-reactive protein(hsCRP) and diabetes and diabetic nephropathy.[Method]According to urine albumin ejection rate(UAER) 106 patients with T2DM were divided into groups with normal urine albumin including 39 cases(NA,UAER < 20ug/ min),with minor urine albumin including 35 cases(MA, UAER20-200ug/ min)and with clinical urine protein including 32(CP,UAER> 200ug/ min)respectively.The concentrations of serum hsCRP of 106 patients with T2DM and 33 patients in the control group were tested by turbidimetry.[Result]The levels of serum hsCRP in T2DM was 3.37±2.83mg/l,which was obviously higher than that of control of 1.25±0.68mg/l(P < 0.01).The levels of NA,MA and CP were 2.12±1.87mg/l、2.92±1.55 mg/l and 5.38±3.72mg/l respectively,which were all higher than that of NC(P < 0.01).There were obvious differences between the level of hsCRP of NA and that of MA(P < 0.01),so was the difference between NA and CP(P < 0.01).The level of hsCRP in CP was significantly higher than that of MA.There were obvious correlations between the level of serum hsCRP and UAER(r = 0.66,P < 0.01).[Conclusion]The level of serum hsCRP increased significantly in patients with T2DM,which had a correlation with UAER and reflected the severity of diabetic nephropathy.
