补肾方剂和氨基胍对糖尿病大鼠纯钛种植体骨结合影响作用的实验研究 被引量:2

摘要 目的:探讨在临床上提高糖尿病患者牙种植体的骨结合率的有效方法。方法:建立糖尿病大鼠骨内种植模型,制作HE染色组织切片,采用光镜和骨计量学参数观察,比较补肾方剂和氨基胍对糖尿病大鼠纯钛种植体骨结合改善作用的差异。结果:注射链脲佐菌素(STZ)3d后大鼠空腹血糖显著升高(P<0.05),尿糖阳性。光镜观察(种植体植入术后12周时):糖尿病补肾方剂治疗组(中药组)结合骨板连续性较好,较厚,表面光滑,周边可见大量的成骨细胞,骨细胞密度较大。糖尿病氨基胍治疗组(西药组):结合骨板比较薄,连续性较好,周边可见大量的成骨细胞,骨细胞密度较大。骨计量学参数比较(种植体植入术后12周时):糖尿病组结合骨板的宽度、种植区成骨细胞数、种植区骨细胞数及非种植区骨细胞数均显著低于西药组、中药组(P<0.05)。西药组结合骨板的宽度、种植区成骨细胞数、种植区骨细胞数均显著低于中药组(P<0.05)。结论:氨基胍可通过控制糖尿病,改善种植体的骨结合;补肾方剂可通过控制糖尿病和调节骨代谢,有效促进和提高种植体的骨结合,其效果优于氨基胍。 Objective: To establish implant model of diabetic rats and compare the effects of tonifying kidney recipes (TKR) and aminoguanidine on the osseointegration of titanium implants, and to provide effective methods to improve the osseointegration of the diabetics. Methods : HE staining were used to observe the bone histomorphometric parameters. Results: Three days after streptozocin injection, increase of blood sugar ( P 〈 0.05 ) showed in pathologic group, with urine sugar positive. Light microscope observation: Twelve weeks after implantation, combined bone lamella relatively thick, continuity fairly good, surface smooth, many osteoblasts around, with fairly high density of osteocytes in TKR group. In the aminognanidine group, the combined bone lamella relatively thin but continuity fairly good, also many osteoblasts seen around, and the density of osteocytes relatively high. Histomorphometric parameters comparison ( 12 weeks after the implantation) : Combined bone lamella width, the numbers of the implant area osteoblasts and osteocytes and the non-implant area osteocytes in diabetic group are smaller than those in TKR and aminoguanidine groups (P 〈 0.05) ; the bone lamella width, the numbers of the implant area osteoblasts and osteocytes in aminoguanidine group significantly smaller than those in TKR( P 〈 0.05 ). Conclusion: Aminoguanidine can improve the osseointegration of titanium implants by controlling diabetes, while TKR can improve the osseointegration of titanium implants by controlling diabetes and modulating bone metabolism more effectively.
