组织多普勒成像技术评价阻塞性睡眠呼吸暂停综合征患者心脏功能 被引量:2

摘要 目的:阻塞性睡眠呼吸暂停综合征(OSAS)患者多合并严重的心血管并发症,因而对于OSAS患者心血管功能异常的早期评价越来越受到人们的重视。文中应用组织多普勒成像(TDI)技术评价OSAS患者心脏功能。方法:OSAS组及正常对照组各35例。应用脉冲波组织多普勒显像(PW-TDI)技术于心尖四腔切面上取样并显示二尖瓣环左心室侧壁侧、室间隔侧及三尖瓣环右心室侧壁侧运动频谱,测量相应取样位置收缩期峰值速度(Sm)及舒张早(Em)、晚期(Am)峰值速度。结果:OSAS组二尖瓣环左心室侧壁侧Sm(P<0.05),二尖瓣环室间隔侧Sm、Em、Em/Am(P<0.05)及三尖瓣环右心室侧壁侧Sm、Em、Am、Em/Am(P<0.01)与正常对照组相应指标比较,均有统计学显著差异。结论:TDI技术可以检测出OSAS患者早期心脏功能异常。 Objective:Patients with obstructive sleep apnea syndrome (OSAS) often had severe cardiovascular complications. So the early detection of the cardiovascular funetional abnormality was more and more thought highly of by people. We evaluated the cardiac function of the patients with (OSAS) using tissue Doppler imaging (TDI) technique. Methods:Thirty-five patients with OSAS and 35 healthy controls were enrolled in the study. TDI was applied in the pulse-Doppler mode to allow for a spectral display and recording of mitral annulus at septal, lateral comer and tricuspid lateral corner in the apical four chamber view. The peak systolic (Sm), early diastolic (Em) and late diastolic (Am) TDI velocity both of mitral and tricuspid annulus were measured by pulsed wave tissue Doppler imaging technique. Results : There were significant differences in Sm of mitral lateral comer, in Sm, Em and Em to Am ratio of mitral septal corner and in Sm, Em, Am and Em to Am ratio of tricuspid lateral corner between the OSAS group and healthy control group (P 〈 0.05 ). Conclusion : TDI technique can detect the early cardiac dysfunction of the OSAS patients with preserved cardiac function evaluated by conventional echocardiography.
