多普勒组织成像技术评价冠状动脉旁路移植术后左心室功能的变化

ASSESSMENT OF THE LEFT VENTRICULAR FUNCTION AFTER CORONARY ARTERY BYPASS GRAFTING USING DOPPLER TISSUE IMAGING

摘要 目的采用多普勒组织成像(DTI)技术评价冠状动脉旁路移植(CABG)术后左心室功能变化。方法随机选择42例CABG病人,应用超声心动图DTI技术,在心尖四腔心切面,记录二尖瓣环侧壁处心肌运动频谱,测量收缩期心肌峰值运动速度(Vs),舒张早期心肌峰值运动速度(Ve)、心房收缩期心肌峰值运动速度(Va),计算Ve/Va比值。结果与术前比较,左心室收缩功能Vs在术后1月显著增加(t=5.91,P<0.01);左心室舒张功能Ve在术后1月显著增加(t=10.91,P<0.01),Va在手术前后变化无显著性意义(t=0.94,P>0.05),Ve/Va比值在术后1月显著增加(t=7.78,P<0.01)。Vs与左心室射血分数(EF)呈正相关(r=0.57,P<0.01)。结论CABG术后由于血运重建,左心室功能明显改善,DTI技术测量二尖瓣环侧壁处心肌运动频谱能够反映CABG术后左心室整体功能的变化。 Objective To assess the left ventricular function in patients before and after coronary artery bypass grafting (CABG) using Doppler tissue imaging (DTI). Methods Using DTI, 41 patients were examined before and after CABG in terms of the systolic peak myocardial motion velocity (Vs), early diastolic peak myocardial motion velocity (Ve), late diastolic peak myocardial motion veloclty (Va) at lateral sides of the mitral annulus in the apical four chamber view. The ratio of Ve/Va was calculated. Results One month after CABG, the Vs, Ve, and Ve/Va increasd significantly (t = 5. 91- 10.91, P〈0.01) compared with before CABG, but the changes of Va were not significant (t=0.94, P〉0.05). Vs correlated positively with EF (r= 0.57, P〈0.01). Conclusion Left ventricular function improved after coronary artery bypass grafting due to revascularization. The lateral sides of the mitral annular motion velocities measured with Doppler tissue imaging can reflect the left ventricular function af ter bypass surgery.
