摘要 目的探讨血液学分析中测量不确定度在血小板计数(BPC)方面的应用。方法参照国际血液学标准化委员会(ICSH)推荐的BPC参考方法计数处理过的全血标本中的血小板(PLT),并利用相关实验数据和文献资料,按照国际标准化组织(ISO)《测量不确定度表述指南》文件中有关测量不确定度的评估方法,采用方差分析法,计算BPC过程中的不确定度。结果本实验研究评价了影响参考方法BPC的各个分量的不确定度,而所占比例最大因素为天间误差因素,其次是瓶间误差因素,天内误差和仪器校准因素所占比例较小;处理过的全血标本中BPC的扩展不确定度(扩展因子k=2)和95%置信区间为(189.6±5.4)×109/L。结论在规定了测量过程和分析样品类型时,不确定度能反映多种因素对检测结果的影响,不确定度能适用于该项目的测量值,有利于实验室间结果的可比性。 Objective To investigate the application of the measurement uncertainty of blood platelet counts (BPC) in haematological analysis. Methods According to the BPC reference method recommended by International Council for Standardization in Haematology (ICSH) , we enumerated platelets in the processed whole blood sample and calculated the uncertainty in this process by method of variance analysis. We use the evaluation methods of measurement uncertainty according to ISO guideline Expression of Uncertainty in Measurement. Results Among different components of BPC uncertainty, the most common one was day-to-day variation, and the second one was vial-to-vial variation. Within day variation and instrument calibration factors took a small part. BPC expanded uncertainty ( expand factor k = 2 ) in the processed whole blood and 95% confidence interval was( 189.6±5.4)×10^9/L. Conclusions After the measure- ment process and sample types are def/ned, uncertainty can reflect the influence of various factors on test results and can be applied to this measurement, which increases the comparability of interlaboratory results.
