摘要
目的探讨Survivin和p53蛋白在结直肠腺瘤转化和发展中的作用。方法采用免疫组织化学方法检测Survivin和p53蛋白在45例结直肠腺瘤中的表达。另选结直肠高分化腺癌和正常结直肠黏膜各10例作为对照。结果45例腺瘤中有15例Survivin阳性表达,其中低级别腺瘤Survivin阳性表达率11%(3/26),高级别腺瘤为63%(12/19);正常黏膜和腺癌Survivin阳性表达率分别为0%(0/10)和90%(9/10)(P<0.01);低级别腺瘤Survivin阳性表达率与高级别腺瘤和腺癌差异均有显著性意义(P<0.01),但与正常结肠黏膜差异无显著性意义(P>0.05);而高级别腺瘤Survivin阳性表达率与正常黏膜和低级别腺瘤差异均有显著性意义(P<0.01),与腺癌也有差异(P<0.05);p53蛋白在低级别腺瘤、高级别腺瘤、正常黏膜和腺癌阳性表达率分别为15%(4/26)、73%(14/19)、0%(0/10)和90%(9/10),其表达模式类似Survivin,差异也有显著性意义(P<0.01或P<0.05);Survivin和p53蛋白表达呈正相关(r=0.52,P<0.01)。结论Survivin和p53在结直肠腺瘤的恶性转化和发展中可能起重要作用,对结直肠腺瘤联合检测Sur-vivin与p53有助于结直肠腺瘤恶性变的早期诊断和生物学行为的评估。
Objective To investigate the expression and significance of Survivin and p53 protein in colorectal adenomas (CRA). Methods Immunohistochemical method by two-step EnVision was used to detect Survivin and p53 protein expression in 45 patients with CRA. The relationship between the expression of Survivin and p53 protein was retrospectively analyzed. Posi- tive control is CRA (Stage III), and negative control is normal colorectal mucosa. Results In 45 patient samples, 15 samples are positive in Surviving expression, among which the positive percentage of Surviving in low-grade CRA is 11% (3/26) , and in high-grade CRA is 63% (12/19). The positive percentage of Surviving in mormal control of colorectal mueosa, and in CRA is 1% (0/10) and 90% (9/10) respectively. There is significant difference in Survivin expression in low-grade CRA control (P 〈 0. 01) , but no difference in Survivin low-grade CRA and normal colorectal mucosa (P 〉 0. 05). However, there is signifi- cant difference in Surviving expression between high-grade CRA group and both normal colorectal mucosa and low-grade CR, A (P 〈 0. O1 ) , and significant difference between high-grade CRA group and positive control ( P 〈 0. 05 ). Positive percentage of p53 protein in low-grade CRA, high-grade CRA, normal colorectal mucosa and positive CRA samples are 15% {4/26) ,73% (14/19) ,0% (0/10) and 90% (9/10) respectively. The positive rates of Survivin and p53 protein in high grade CRA were all higher than those in low grade CRA and matched normal tissues ( P 〈 0.01 or P 〈 0. 05 ). Possitive correlation was also found between Survivin and p53 protein expression ( r = O. 52, P 〈 0. 01 ). Conclusion Survivin and p53 protein can be an impor- tant role for the transformation from adenoma to adenocareinoma. Combined examination of Survivin and p53 protein will be helpful to evaluate the transformation from adenoma to adenocarcinoma and early diagnosis of colorectal adenoma and evaluation of its b
