床旁胸片对重症监护室机械通气患者的诊断价值 被引量:4

摘要 目的研究重症监护室(ICU)机械通气患者床旁胸片的影像特点,分析其与机械通气呼气末正压通气(PEEP)值的关系。方法回顾性分析91例接受机械通气患者的床旁胸片及临床资料。结果91例接受机械通气患者,床旁胸片表现为炎症者43例(47.25%),肺水肿1例(1.10%),不能确定者3例(3.30%),两者并存26例(28.57%),急性呼吸窘综合征(ADRS)8例(8.79%),其他患者10例(10.99%);床旁胸片表现与PEEP值同步变化者3例。不同步变化者73例,无法比较者15例。结论床旁胸片对ICU机械通气患者有很高的诊断价值。 Objective To study the radiographic features on portable chest radiographs in mechanically ventilated patients in ICU,and to analyze their relations with the positive end expiratory pressure(PEEP).Methods Portable chest radiographs and the associated clinical records of 91 patients took mechanical ventilation in ICU were reviewed retrospectively. Results After interpreting the chest films,pneumonia,pulmonary edema,amphibolous,concurrence,acute respiratory dis- tress syndrome(ARDS)and other clinical phenomenons were diagnosed separately in 43 patients(47.25%),1 case (1.10%),3 patients(3.30%),26 patients(28.57%),8 patients(8.79%)and 10 patients(10.99%).In3 patients,the chest films and the PEEP changed simultaneously,while in 73 patients the PEEP changed,but the chest films remained,and the changes could not be compared in the rest 15 patients.Conclusion Portable chest radiographs have very importment diagnostic value to mechanically ventilated patients in ICU.
