16层螺旋CT双期3D-CTA对脑膜瘤术前评估的价值 被引量:18

摘要 目的评估16排螺旋CT双期三维血管造影(3D-CTA)显示脑膜瘤与邻近血管关系的能力。方法回顾性分析61病理证实的脑膜瘤术前双期3D-CTA影像资料,与手术结果进行对照,评估3D-CTA显示脑膜瘤与周围脑血管关系的能力。结果以手术为标准,本研究对42例与临近大动脉接触的脑膜瘤CTA判定为A型(肿瘤未侵蚀血管)28例,24例与术中发现一致(85.7%),CTA判定为B型(肿瘤侵蚀血管)14例,12例与术中发现一致(85.7%)。对28例靠近静脉窦的脑膜瘤,CTA判定有临近静脉窦侵犯的共18例,全部被手术证实;其中14例CTA判断其侵犯程度与手术一致。结论双期3D—CTA提供了脑膜瘤与邻近大血管、静脉窦及骨骼三维空间准确关系;可明确区分肿瘤周围的动静脉成分;有利于手术方案的制定及提高术中处理的可预测性。 Objective To: explore the role of double-phase 3D 16:slice MDCT angiography(MDCTA) in the preoperative evaluation of intracranial vascular involvement induced bymeningiomas. Methods The preoperative double-phase 3D-CTA image data of 61 pathologically confirmed meningioma was analyzed retrospectively. The results were compared with intra.operative findings. The ability of the relationship between rneningioma and its Surrunding vessels with 3D-CTA was evaluated. Results Regarded the surgical findings as standard, this study of 42 patients with artery close contact with the meningiomas; CTA revealed that 28 cases were type A ( surrounding arteries remained intact ), 24 of which were consistent with the surgical findings: 14 cases were type B (erosion of artery)12 of WhiCh were consistent with the surgical findings. Of the 28 cases around the dural sinus meningiomas, CTA determine 18 cases invaded the sinus, all of thel8 cases were confirmed completely by the surgery, and invasion degree judged by CTA was consiStent with surgical finding in 14 cases. Conclusion Doublephase 3D MDCT angiography (MDCTA) can provide accurate anatomic information regarding the lesion and its surrounding vessels and bony structures and demonstrate lesions, arteries, and veinS, separately, aid in surgical planning and improve the predictability in the surgical management.
