手指中节指骨骨折切开复位内固定治疗方式的改良 被引量:27

The modified open reduction and internal fixation for treatment of middle phalangeal fractures

摘要 目的研究分析应用切开复位微型钛板侧方固定治疗中节指骨骨折的方式,并探讨其疗效。方法对10例10处中节指骨骨折,行侧方切开复位后,保护并维持指背伸肌装置的完整,采用微型钛板侧方固定治疗。术后2d开始屈、伸功能锻炼。结果术后随访3~8个月,平均5个月。10例均于术后4—8周(平均6周)达到骨折临床愈合,无畸形愈合。按TAM法进行功能评定:优8例,良2例。结论采用微型钛板侧方固定治疗手指中节指骨骨折,既可维持指背伸肌腱辅助结构的完整,又可减少术后伸肌腱粘连和磨损,手指功能恢复好。 Objective To study and analyze the outcome of modified open reduction and internal fixation using mini titanium plate for treatment of middle phalangeal fracture. Methods Ten cases (ten fractures) of middle phalangeal fractures were treated by open reduction and internal fixation. After exposure of the fracture site and protection of the extensor apparatus via a lateral approach, the fracture was fixed with mini titanium plate and screws. Early postoperative rehabilitation started 2 days after the operation. Results Postoperative follow up ranged from 3 months to 8 months, with an average of 5 months. The clinical healing time ranged from 4 to 8 weeks and averaged 6 weeks. Eight of the cases were rated as excellent and 2 good according to the TAM scores. Conclusion The modified lateral internal fixation using mini titanium plate for treatment of middle phalangeal fractures can avoid disturbing the extensor apparatus, reduce tendon adhesion and abrasion, and achieve excellent functional recovery of the injured finger.
