摘要
背景:同种异体骨移植后的免疫排斥是异体骨移植必须面对的重要问题。目前关于大段同种异体下颌骨移植重建下颌骨缺损以及移植造成的免疫研究报道很少见。目的:建立犬自体/同种异体下颌骨移植重建颞下颌关节、下颌骨缺损的动物模型,观察动物骨移植前后外周血白细胞介素2、白细胞介素4、白细胞介素6水平,了解异体下颌骨移植引起的免疫反应。设计:动物模型建立为随机对照动物实验,白细胞介素水平测定为自身对照。实验于2004-03/2005-04在解放军总医院动物实验中心完成,检测工作在解放军总医院第二附属医院核医学科完成。材料:健康杂种犬18只制作包括髁突在内的下颌骨升支部分缺损的杂种犬动物模型。健康成年比格供体犬头18具,随机分成2组,每组9块,分别进行冷冻和冻干处理。方法:取杂种受体犬18只制作包括髁突在内的下颌骨升支部分缺损的杂种犬动物模型,随机分成3组:自体骨移植组、异体冷冻骨移植组、异体冻干骨移植组,每组6只。主要观察指标:术前,术后2,4,8,16,24周检测外周血白细胞介素2,白细胞介素4、白细胞介素6水平。结果:移植术后所有动物均顺利存活,伤口愈合良好。自体骨移植组术后2周的白细胞介素2水平明显高于术前(P<0.05);异体冷冻骨移植组术后2周的白细胞介素2、白细胞介素6水平明显高于术前(P<0.05),术后2,4,8周白细胞介素4水平明显低于术前(P<0.05);异体冻干骨移植组术后2周的白细胞介素2、白细胞介素6的水平明显高于术前(P<0.05),其余时间与术前无明显差别。结论:犬冷冻/冻干异体下颌骨移植可以达到临床愈合,不会引起外周血白细胞介素水平的持续异常。
BACKGROUND: The immune response of allograft bone is a very important problem, but the reports on massive allograft mandible transplantation for mandible defect reconstruction are rare. OBJECTIVE: To explore the immune responses after transplantation of mandible allografts in caninies through peripheral blood intedeukin-2 (IL-2), IL-4, and IL-6 in animal models. DESIGN: A randomized controlled animal trial was performed at the Animal Experimental Center, General Hospital of Chinese PLA and the Department of Nuclear Medicine, Second Hospital of General Hospital of Chinese PLA between March 2004 and Arial 2005. MATERIALS: Eighteen healthy dogs were used to make animal model of mandible defect. Eighteen mandibles of Beggles were used as allograft mandible, and randomly divided into two groups (n=9) and accepted frozen and freeze-drying. METHODS: Eighteen donor dogs were randomly divided into three groups (n=6): autogenous mandible, frozen allograft mandible, and freeze-drying allograft mandible groups. MAINOUTCOME MEASURES: Peripheral blood samples were obtained for radioimmunoassay of IL-2, IL-4 and 1L-6 at weeks 2, 4, 8, 16 and 24 postoperatively. RESULTS: All animals survived, with no wound infection postoperatively. IL-2 in autogenous mandible group increased significantly 2 weeks after transplantation compared with before surgery (P 〈 0.05). IL-2 and IL-6 increased significantly 2 weeks postoperatively while IL-4 decreased significantly 2, 4, 8 weeks postoperatively in frozen allograft mandible group (P 〈 0.05). IL-2 and IL-6 increased significantly 2 weeks after transplantation in freeze-drying allograft mandible group (P 〈 0.05), and at differences were found at other time points. CONCLUSION: The frozen and freeze-drying bone allografts can achieve clinical healing, and do not lead to consistent abnormality of IL-2, IL-4 or IL-6 of host peripheral blood.
