视觉电生理检测在原发性开角型青光眼中的临床研究 被引量:6

摘要 目的:将视觉电生理应用于开角型青光眼的早期诊断,探索一种有效、敏感及简便的青光眼诊断方法。方法:对可疑青光眼60例(105眼)、正常对照30例(60眼)进行图形视网膜电图(P-ERG)、图形视诱发电位(P-VEP)、振荡电位(OPs)各项指标的检测;对有以上指标异常的66眼进行分组,干预组34眼,观察组32眼,干预组局部用药,眼压下降至患者基础眼压的30%。结果进行统计学分析。所有研究对象追踪6~12mo。结果:可疑青光眼组P-ERG幅值降低及潜伏期延长、OPs波总值降低和P-VEP幅值降低及潜伏期延长,阳性率分别为57.1%,33.3%,47.6%,两项以上异常为36.1%,联合检测的阳性率为70.6%。6mo追踪随访各指标再进行组间对照,P-VEP幅值有显著差异。12mo追踪随访各指标再进行组间对照,OPs的总值有显著性差异,P-VEP的潜伏期有显著性差异。结论:视觉电生理中的(P-ERG,P-VEP,OPs)检测在可疑青光眼患者早期诊断中有临床意义。经过6~12mo的追踪检查在所有的指标中P-VEP潜伏期、幅值及OPs总值比较敏感。因此视觉电生理检查可作为原发性青光眼早期诊断的重要手段,并可作为观察病情的监测指标。 AIM: To explore a new, effective, rapid, simple, and sensitive method for the early diagnosis of primary openangle glaucoma by applying visual electrophysiology. METHODS: Pattern electroretinogram (P-ERG), pattern reversal visual evoked potential (P-VEP) and oscillatory potentials (OPs) were tested on 60 glaucoma suspects (105 eyes) and 30 normal subjects (60 eyes) respectively. Grouping intervention was performed on the eyes with the above abnormal indexes: intervention group and control group. The intervention group (n =34) local were performed local medication until lOP was reduced to 30% normal intraocular pressure. In control group (n = 32), the patients were not performed the guidance as aboved. Use statistics method to analyze the results. All the cases were followed up for 6-12 months. RESULTS: The amplitude of the pattern electroretinogram decreased and the latencies of wave peak of pattern electroretinogram were prolonged in all sixty glaucoma suspects. The total value of oscillatory potentials decreased in all sixty glaucoma suspects. The amplitude of the pattern reversal visual evoked potential decreased and the latencies of wave peak of pattern reversal visual evoked potential were prolonged in all sixty glaucoma suspects. The positive ratio was 57.1%, 33.3% and 47.6% respectively. Two positive out of the above mentioned three tests was 36. 1%. The positive ratio of combined detection was 70.6%. Statistics analysis was conducted among every index after 6 months and the amplitude of the pattern electroretinogram showed significant difference. Statistics analysis was conducted among every index after 12 months and the total value of oscillatory potentials and the latencies of wave peak of pattern reversal visual evoked potential showed significant difference. CONCLUSION: The tests of P-ERG, P-VEP and OPs are of great clinical significance in the diagnosis of patients with suspect glaucoma.
