摘要
目的探讨结直肠损伤的特点和结肠损伤Ⅰ期修复的适应证以及结直肠损伤的处理方法。方法回顾性分析1997年5月—2007年5月收治的结直肠损伤25例临床资料。结果25例行急诊手术,20例(80%)行Ⅰ期手术,其中单纯肠管修补14例,切口感染2例;6例行破裂结肠部分切除吻合术,吻合口漏1例,切口感染1例。2例行乙状结肠双腔造口+局部引流术,切口感染1例。结论了解结直肠损伤的特点,提高结直肠损伤的认识,掌握好结肠Ⅰ期修复的适应证、直肠损伤处理的特殊性是减少结直损伤并发症,提高外科治疗效果的关键。
Objective To explore the characteristic of colorectal trauma and the indication for one- stage repair and the managed method. Methods The clinical data of 25 patients with coloreetal trauma from May 1997 to May 2007 were retrospectively analyzed. Results Emergency operations were performed on 25 patients, primary repair or primary anastomosis after excision were performed in 20 cases （80%）, and 2 cases of incisional infection were occurred in 14 cases of primary repair. Primary anastomosis after excision was performed in 6 cases, infection occurred in 1 ease and 1 case of anastomotic leakage. Double luminal sigmoidostomy was performed in 2（8 %） and incision infection was occurred in 1 case. Conclusion It is important to understand the characteristics of colorectal trauma, grasp indications for one- stage operation, and specificity of management of rectal trauma and iatrogenic colorectal trauma for decreasing the complication and improving the effect of surgical treatment.
