彩色多普勒血流显像技术监测肝移植术后发生血管并发症的时间分析 被引量:5

Monitoring vascular complications following liver transplantation using color Doppler flow imaging

摘要 目的探讨彩色多普勒血流显像(CDFI)技术在监测肝移植术后发生血管并发症时间分布的意义。方法对792例肝移植患者在术后不同时间进行CDFI检查,并对CDFI诊断血管并发症的患者进行血管造影、螺旋CT检查或手术确诊。结果在792例肝移植患者中,发生血管并发症共54例,发生时间在术后1~360d,其中在1~30d、31~60d、61-90d、91~180d和181~360d发生血管并发症的构成比分别为46.3%(25/54)、22.2%(12/54)、14.8%(8/54)、9.3%(5/54)和7.4%(4/54)。其中肝动脉并发症、门静脉并发症及流出道梗阻的构成比分别为61.1%(33/54)、35.2%(19/54)和3.7%(2/54)。结论大多数血管并发症发生在术后6个月,肝移植术后连续密切CDFI监测有利于早期发现血管并发症。 Objective To assess the value of color Doppler flow imaging (CDFI) in monitoring vascular complications following orthotopic liver transplantation (OLT). Methods Seven hundred ninty- two patients who received OLT from April 2002 to December 2006 in the Organ Transplantation Center, General Hospital of Chinese People's Armed Police Forces, Beijing, and underwent CDFI examinations in different periods after OLT were enrolled in this study. Their vascular complications were monitored by CDFI and confirmed by angiography or spiral CT. Results Of the 792 patients, 54 were diagnosed with vascular complications that occnred 1-360 days after their OLT operations. These complications occurred within 1-30 days, 31-60 days, 61-90 days, 91-180 days, 181-360 days, with the proportions of 46.30%, 22.22%, 14.81%, 9.26% and 7.41% respectively. The proportion of hepatic artery and portal vein compli- cations and outflow occlusions were 61.11%, 35.19% and 3.70% respectively. Conclusion Most vascular complications occurred within six months after the OLT operation. The continuous and careful monitoring by CDFI is beneficial in an early diagnosis of vascular complications after OLT.
