Efficacy of alpha-lipoic acid in treating diabetic peripheral neuropathy in patients with type 2 diabetes mellitus

摘要 目的评价α-硫辛酸对2型糖尿病周围神经病变(DPN)的疗效。方法45例DPN的患者随机分为两组,治疗组23例,给予α-硫辛酸600 mg加液体静脉点滴,1次/d,共2周;对照组22例,给予甲钴胺1 000μg加液体静脉点滴,1次/d,共2周。观察临床症状和体征变化及神经传导速度(NCV),检测血C反应蛋白(CRP)、谷胱甘肽、尿微量白蛋白。结果治疗组有效率达91.3%,明显高于对照组的63.6%(P<0.05);治疗后两组神经传导速度均较治疗前明显改善(P<0.01)。结论α-硫辛酸治疗DPN,疗效显著。 Objective To investigate the effects of alpha - lipoic acid in treating diabetic peripheral neuropathy (DPN) in patients with type 2 diabetes mellitus. Methods Forty - five subjects were assigned randomly to two groups and treated respectively with alpha - lipoic acid or mecobalamin intravenously for 2 weeks. The changes in their symptoms and signs were observed, the motor and sensory nerve conduction velocities (MNCV, SNCV) of median nerve and common peroneal nerve were detected with electromyography, and the plasma level of C reactive protein(CRP) and glutathione as well as microalbumineeria were evaluated before and after the treatment. Results The total effective rate of therapy group was 91.3%, significantly higher than that of control group (63.6% , P 〈 0.05 ). There were significant decrease in microalbuminceria and CRP level ( P 〈 0. 01 ). Conclusion Antioxidant alpha- lipoie acid could normalize the status of oxidative stress and accelerate the nerve conduction velocity and improve the symptoms of DPN with an effect similar to meeobalamin.
