摘要
目的探讨急性期脑梗死血清尿酸增高与脑血管病危险因素的关系。方法比较伴血清尿酸增高的脑梗死组(HUA组)与不伴血清尿酸增高的脑梗死组(NUA组)的临床资料与生化指标和炎症细胞因子的差异,分析血清尿酸增高与脑梗死危险因素,炎症细胞因子的关系及其对临床神经功能缺损程度评分(scores)的影响。结果HUA组患者的WHR、SBP、DBP、FPG、LDL-C、UA、TNFα-、IL-6、IR及Sores均显著高于NUA组患者的水平,P<0.05或<0.01。HUA组血清HDL-C显著低于NUA组水平,P<0.05。多元线性回归分析显示HUA组的WHR、DBP、FPG、HDL-C、LDL-C、TNF-α、IL-6、UA、IR是Scores的主要影响因素。结论急性期脑梗死血清尿酸增高可引起脑梗死危险因素的聚集和炎症细胞因子的升高,共同参与脑梗死的病理生理过程,导致神经功能严重损伤。
Objective To explore the relationship between increase of serum uric acid and danger factors for cerebrovascular disease in patients with acute cerebral infarction. Methods Clinical data, biochemical indicator and inflammatory cytokines in cerebral infarction with high uric acid(HUA) and normal uric acid(NUA) were compared. Analysis was performed to observe the effect of nerve function deficit score (scores) between hyperuricemia, danger factor of cerebral infarction and inflammatory cytokines. Results The levels of WHR,SBP,DBP,FPG,LDL-C,TNF-α,IL-6,IR and scores in patients of HUA group were significantly higher than those of NUA group(P 〈 0.05 or 〈 0.01 ). The levels of serum HDL- C in patients of HUA group were significantly lower than those of NUA group ( P 〈 0.05 ). Multiple linear regression analysis showed the levels of WHR, DBP,FPG, HDL-C, LDL-C, TNF-α, IL-6, UA and IR in patients of HUA group were major forces to scores. Conclusion High uric acid in serum in patients with acute cerebral infarction can cause danger factors of cerebral infarction assemble and ricen of inflammatory cytokines, and involve in the process of cerebral infarction' s pathophysiology,lead to injury of neurological function.
