脑电生物反馈治疗不同亚型注意缺陷多动障碍患儿效果比较 被引量:7

Effect comparison of EEG biofeedback treatment on children with different subtype of attention deficit hyperactivity disorder

摘要 目的:比较脑电生物反馈治疗对注意缺陷多动障碍(ADHD)不同亚型的疗效。方法:选择ADHD患儿103例,其中注意缺陷组34例、多动-冲动组32例及混合组37例,采用Conners量表、韦氏儿童智力测试和TOVA量表分别于治疗前、治疗后(治疗3个疗程后)、治疗后6个月及12个月进行评定。结果:3组Conners量表各因子评分治疗后较治疗前均降低(P<0.05或<0.01),12个月注意缺陷组和混合组仍有下降趋势(P<0.05或0.01),而多动-冲动组较治疗前下降不明显(P>0.05);除注意缺陷组错认评分外(P<0.05),3组TOVA 4项操作评分治疗后与治疗前比较,差异均无统计学意义(P均>0.05),12个月注意缺陷组4项评分、多动-冲动组遗漏项评分、混合组除遗漏项外其他3项评分较治疗前降低(P<0.05或0.01);3组韦氏儿童智力测验c因子评分治疗后较治疗前均升高(P均<0.01),12个月注意缺陷组和混合组与治疗前相比差异有统计学意义(P<0.01),而多动-冲动组与治疗前比较差异无统计学意义(P>0.05)。结论:脑电生物反馈对ADHD不同亚型的疗效是不同的;治疗前首先应进行分类诊断以确定不同的亚型。 Aim: To study the effects of EEG biofeedback treatment on children with different subtype of attention deficit hyperactivity disorder(ADHD).Methods:A total of 103 children with ADHD were divided into three groups by different ADHD subtype in the DSM-Ⅳ.Every group was followed up for 1 year after treatment.Conners,C-WISC,and TOVA were used to assess the efficacy.Results: Significant difference in each variable of Conners and factor c of C-WISC were found among three groups after the treatment(P〈0.05 or 0.01),but the ADHD-HI group had no significant difference at the end of 1 year(P〉0.05).Most of TOVA scores significantly decreased at the end of 1 year in ADHD-I group and ADHD-C group(P〈0.05 or 0.01),but only one score significanly decreased in control ADHD-HI group(P〈0.05).Conclusion: EEG biofeedback therapy has different effective on different subtype of ADHD.The subtype should be definited before EEG biofeedback therapy.
