Significance and Expression of Plasma Adhesion Molecule P-Selecting in Children with Different Glomerulonephritis

摘要 目的探讨不同肾小球疾病患儿外周血黏附分子P选择素(CD62P)的水平及其临床意义。方法选择不同肾小球疾病患儿154例为病例组。其中原发性肾病综合征(PNS)54例,IgA肾病(IgAN)32例,紫癜性肾炎(HSPN)26例,狼疮性肾炎(LN)32例,乙肝相关性肾炎(HBV-GN)10例。病例均处于活动期。42例年龄、性别相匹配的健康儿童为健康对照组。采用放射免疫法(RIA)测定各组外周血CD62P水平,并分析其与患儿24h尿蛋白定量、血小板(PLT)、血清清蛋白(ALB)、胆固醇(CHOL)、BUN、Scr和尿N-乙酰-氨基葡萄糖苷酶(NAG)、β2微球蛋白(β2-MG)、溶菌酶(Lys)之间的相关性。结果1.各病例组活动期患儿外周血CD62P水平均明显增高,与健康对照组比较均有显著性差异(Pa<0.01)。2.各病例组中,PNS组患儿CD62P水平增高最为显著,IgAN组患儿CD62P水平显著低于其他各组(Pa<0.05)。3.各病例组患儿外周血CD62P水平均与24h尿蛋白定量呈正相关,与血清ALB呈负相关,与血清PLT、CHOL、BUN和Scr均无相关性。4.各病例组患儿外周血CD62P水平与尿NAG、β2-MG水平均呈正相关,与尿Lys水平无相关性。结论CD62P在肾小球疾病患儿外周血中表达增强,提示其参与了儿童肾小球疾病的发生和发展。 Objective To investigate the plasma level and clinical significance of adhesion molecule P - selecting (CD62P) in children with different glomerulonephritis. Methods One hundred and fifty - four children with different glomerulonephritis were enrolled in the study, among which 54 cases with primary nephritic syndrome ( PNS), 32 cases with IgA nephropathy (IgAN), 26 cases with Henoch - Schonlein purpura nephritis (HSPN), 32 cases with lupus nephritis (LN) and 10 cases with hepatitis B virus associated glomelalonephrjtis (HBV - GN). All children were at active stage. Forty- two of normal age and sex -matched children served as controls. The level of plasma CD62P were assayed by radioimmunoassay (RIA), and its correlation with 24 hour proteinuria, blood platelet (PLT), serum albumin (ALB), cholesterol (CHOL) , urea nitrogen (BUN) , serum ereatinine (Set) , urine N - acetyl - β - D - glucosaminidase ( NAG ) , urine β2 - micro-globulin (β2 - MG) and lysozyme (Lys) were also analyzed. Results 1. The plasma level of CD62P was all higher in different glomerulonephritis groups than those in control group. 2. Among different glomerulonephritis groups, the plasma level of CD62P was the highest in the group of children with PNS, and the lowest level in the group with IgAN. 3. The plasma level of CD62P in children with different glomendonephritis was positively correlated with that of 24 hour proteinuria, and negatively correlated with serum albumin. No correlation had been found between CD62P and PLT count, serum CHOL, BUN or Scr. 4. The plasma level of CD62P in children with different glomerulonephritis was positively correlated with that of urine NAG and β2 - MG, no correlation had been found between CD62P and urine Lys. Conclusion The increased expression of CD62P in peripheral blood is associated with the occurrence and prognosis of children with different glomerulonephritis.
