摘要 目的:探讨肠功能恢复汤联合肠内营养对腹腔感染大鼠肠黏膜屏障的影响。方法:健康SD大鼠60只随机分为4组:假手术组、肠外营养(PN)组、肠内营养(EN)组、肠功能恢复汤加EN组,分别在24h、48h、3d、5d、7d采集血液检测血清中TNF-α含量。于实验第7d处死,光镜和电镜下观察各组大鼠肠黏膜形态学变化;通过免疫组织化学方法,检测各组肠黏膜上皮细胞occludin蛋白表达情况。结果:PN组大鼠死亡率较其他组高;其余三组肠转运功能较对照组明显降低,PN组小肠黏膜萎缩明显,电镜肠功能恢复汤加EN组肠粘膜比其他组好。各组血清TNF-α含量术后均较对照组升高,48h达峰值。结论:肠功能恢复汤联合肠内营养支持治疗能减轻腹腔感染后肠黏膜损伤,保护肠黏膜屏障,降低细菌移位率。 Objective To investigate the effect of combining the decoction to help recovering intestinal function with enteral nutrition(EN) on function of intestinal mucosa barrier of rats with abdominal cavity infection. Methods Sixty SD rats were divided into four groups randomly: control group, EN group,parenteral nutrition(PN) group, and decoction + EN group.The content of TNF- α in blood serum was detected at 24h,48h,3d,5d,7d. The rats were killed in the seventh day. The mucosa pathology changes of ileum with light microscope and electronic microscopy and the expression of occludin protein in intestinal mucosa epithelium cells were observed. Results The morality of PN group was the high- est among all groups. The motor function in experimental groups were weaker than in the control group. The intestinal mucosa epithelium cells of the PN group army sparsely, the intestinal mucosa epithelium formation of the Decoction + EN group was better than in EN group and PN group. The serum TNF - α level of experimental groups was higher than in control group on postoperatively with a peak at 48 hours. Conclusion Combining the decoction to help recovering intestinal function with enteral nutrition can protect the intestinal mucosal barrier from damage, reducing the injury of intestinal mucosa and the rate of bacterial translocation.
