摘要
目的了解CK7、CK20和villin在肿瘤细胞中的表达情况,探讨联合应用这3个抗体判断原发肿瘤的意义。方法采用组织芯片和免疫组化方法,观察CK7、CK20和villin在392例肿瘤组织(其中肺癌87例、胃癌72例、直肠腺癌36例、肾癌75例、前列腺癌22例、乳腺癌65例、卵巢癌35例)中的表达特点。结果CK7、CK20和villin3种抗体在不同器官来源的肿瘤中的表达可以组合成8种模式:①CK7+/CK20-/villin+:胃腺癌(41.67%)阳性率较高;②CK7+/CK20-/villin-:乳腺浸润型小叶癌(100%)、肺腺癌(95.83%)、乳腺浸润型导管癌(78.57%)、卵巢浆液性腺癌(70.37%)和嫌色细胞癌(66.67%)阳性率较高;③CK7+/CK20+/villin+:肿瘤阳性率均不高;④CK7+/CK20+/villin-:肿瘤阳性率均不高;⑤CK7-/CK20-/villin+:乳头状肾细胞癌(43.47%)阳性率较高;⑥CK7-/CK20-/villin-:肺小细胞癌(100%)、肾透明细胞癌(97.67%)、肺鳞癌(88.57%)和前列腺癌(86.36%)阳性率较高;⑦CK7-/CK20+/villin+:直肠腺癌(52.78%)阳性率较高;⑧CK7-/CK20+/villin-:肿瘤阳性率均不高。结论CK7、CK20和villin联合表达模式有助于判断部分腺癌的器官来源,缩小转移癌原发灶的搜寻范围。
Objective To investigate the expression of CK7, CK20 and villin in tumor cells, and to summarize a valuable regular pattern to find their organic source. Methods 392 cases of tumor tissue were studied by tissue array technology and immunohistochemistry for CK7, CK20 and villin. Results There were 8 kinds of combined patterns of CK7, CK20 and villin in different cancers: (1) CK7 +/CK20 -/villin + : gastric adenocarcinoma (41.67%) ; (2) CK7 +/CK20 - / villin - : breast invasive lobular carcinoma ( 100% ), pulmonary adenocarcinoma ( 95.83% ), ovarian serous adenocarcinoma ( 70.37% ), pulmonary adenocareinoma (95.83%), breast invasive ductal carcinoma (78.57%), ovarian .serous adenocarcinoma (70.37%), chromophobe renal cell carcinoma (66.67%), has a high positive rate; (3) CK7 +/CK20 +/villin + : none of these has a high positive rate; (4) CK7 +/CK20 +/villin - : none of these has a highe positive rate; (5) CK7 -/CK20 -/villin + : papillary renal cell carcinoma (43.47%) has a high positive rate; (6) CK7 -/CK20 -/villin - : pulmonary small cell carcinoma (100%), renal clear cell carcinoma(42/43, 97.67% ), pulmonary squamous cell carcinoma(88.57% ), prostatic carcinoma (86.36%), have high positive rate; (7) CK7 -/CK20 +/villin + : rectal adenocarcinoma (52.78%), have high positive rate; (8) CK7 -/CK20 + /villin - : none of these tumors has a high positive rate. Conclusion The expression pattern of CK7, CK20 and villin in defferent carcinomas is helpful for determining the origin of some metastatic adenocarcinomas, and minimizing the range of origin of metastatic adenocarcinomas, and establishing the groundwork about seaching for the origin of adenocarcinomas.
