摘要 目的研究解脲脲原体(Uu)在健康幼儿泌尿生殖道中的检出情况与分群分型,以探讨Uu在该人群泌尿生殖道中的分子流行病学情况。方法收集400例健康幼儿尿液,将离心后的尿沉渣直接接种于Uu液体选择性培养基进行初筛,初筛阳性的标本用聚合酶链反应(PCR)进行分群。对Uu生物一群的标本再用PCR法及酶切法进一步分型。结果400例健康幼儿中,Uu液体培养阳性36例,PCR扩增阳性32例(8.00%),4例未扩增。其中单纯生物一群23例(71.88%),单纯生物二群7例(21.87%),两群混合2例(6.25%)。生物一群中1型7例(21.88%),6型11例(34.38%),3/14型4例(12.50%),混合型1例(3.12%)。252例男婴中阳性12例(4.76%),148例女婴中阳性20例(13.51%),两组阳性率差异有统计学意义(χ2=9.70,P<0.01)。结论Uu在健康幼儿泌尿生殖道中的携带率为8.00%,以生物一群为主,主要血清型是1型和6型;女性幼儿检出率高于男性幼儿。在健康幼儿泌尿生殖道中,Uu可能属于一种共生微生物。 Objective To determine the isolation rate and serotyping of Ureaplasma urealyticum (Uu)in urogenital tract of healthy infants, and to discuss molecular epidemiology of Un in this population group. Methods Urine specimens were collected from 400 infants, and all specimens were cultured by commercial Uu selective liquid medium. The positive cases were then clarified into biovars and serotypes by PCR method and restriction enzyme excision. Results Four hundred healthy infants were screened and 36 cases were found to be positive by Uu culture,32 were amplified and 4 were unamplified by PCR, and the positive rate of Uu in healthy infants was 8.00% .In 32 cases, 23 were Parvum biovar (71.88 % ). 7 were Urealyticum biovar(21.88 % ), 2 were found to be mixture of two biovars (6.24 % ). In Parvum biovat,7 cases(21.88% ) ,11 cases (34.38%) and 4 cases(12.50% ) were of serotype 1,6,3 and 14 respectively.The mixed isolations of these four serotypes were in 1 case (3.12 % );4.76 % of male infants were positive for Uu(12/252), and 13.51% of female infants were positive for Uu(20/148). The isolation rate of Uu between the two groups had significant difference (χ^2 = 9.70, P 〈 0.05). Conclusions Uu can be isolated from lower urogenital tracts of healthy infants, and the isolation rate is 8.00 %. The main isolation is Parvum biovar. The isolation rate of Uu in female infants is higher than in male ones. Uu colonized in lower urogential tract may be a common symbiotic organism.
