缝线拉网颗粒状植骨椎管扩大成形的实验研究

摘要 目的:探讨全椎板切除后在缝线拉网保护下颗粒状植骨椎管扩大成形的可行性。方法:12只家兔,CT测量L1椎管矢状径。切除L1全椎板,利用周围关节突及棘突为附着点,缝线拉网,植自体颗粒状骨。术后2周及12周CT测量L1椎管矢状径。通过大体及病理切片观察椎管成形情况。结果:L1椎管矢状径术前为3.54±0.26mm,术后2周为8.39±0.63mm,较术前增大,无骨块进入椎管,术后12周为3.57±0.29mm,与术前无统计学差异。术后12周植入的颗粒状骨部分被吸收,残存部分互相融合成骨板样位于缝线网上方,缝线网无松脱及断裂,网下原椎板切除后的缺损被不规则再生椎板填充。结论:全椎板切除后缝线拉网简便易行,结合颗粒状植骨可行兔椎管扩大成形;兔的全椎板再生能力较强,用其制作椎管扩大成形的动物模型不理想。 Objective:To study the feasibility of vertebral canal reconstruction by grafting granular bone on a suture net weaved behind the dura after laminectomy.Method:3D CT scan was completed in 12 Chinese rabbits to measure the sagittal diameter of L1 vertebral canal. Laminectomy of L1 was completed in all the rabbits, a suture net was weaved behind the dural sac by 1# suture line, which stuck to the articular processes and the spinous processes, some little granular bone, which was got from its own iliac bone, was laid on the net. 3D CT scan was taken again 2 and 12 weeks after operation respectively to measure the sagittal diameter of L1 vertebral canal.Then all the animals were sacrificed to observe the state of vertebral canal reconstruction in gross specimen and the pathological sections. Result:The sagittal diameter of L1 was 3.54±0.26mm before operation, it was 8.39±0.63mm and 3.57±0.29mm at the 2nd and 12th week after operation respectively,it was enlarged at the 2nd week (P〈0.05), but no obvious change at the 12th week (P〉0.05). There was no granular bone be found fallen into the canal from CT scan. At the 12th week after operation, part of the grafted granular bone was absorbed, the remains fused as a plate on the net, and new laminae reproduced under the net shown in the gross specimen and pathological section.Conclusion :It's convenient to weave a suture net, and it's feasible to enlarge the verteberal canal of rabbits by grafting granular bone on the net. The rabbit is not fit for the animal model studying of vertebral canal reconstruction because of its greater of laminal regeneration ability.
