从中医学角度认识脑缺血损伤后内源性神经干细胞的增殖分化 被引量:10

摘要 通过对脑缺血损伤后内源性神经干细胞增殖分化研究的回顾与分析,从中医学角度对此现象进行了思考,认为肝肾精血盛衰是神经干细胞(NSCs)增殖分化的物质基础,先天之精、肾精、督脉、脑髓、神经干细胞增殖分化有密切联系;提出了毒邪害络损髓是抑制脑缺血后内源性神经干细胞增殖分化的病理机制,脑缺血急性期,痰瘀热毒混杂脑内,损伤脑髓、督脉、督络,是抑制NSCs分化增殖主要因素,清热化痰、祛瘀解毒通络是治疗大法;恢复期肝肾精血亏虚,毒邪留恋,NSCs增殖乏源是主要病理因素,治疗当以补益肝肾为主,解毒通络为辅。 With retrospect and analysis on the research done on the proliferation and differentiation of endogenous neural stem cells following cerebral ischemia, the authors believe that the condition of the essence blood in the liver and kidney is the material basis for the proliferation and differentiation of neural stem cells (NSCs). The congenital essence, kidney essence, Du Meridian, the brain marrow and the neural stem cells are closely related to each other. The authors put forward the view that the pathogenesis for the inhibition of proliferation and differentiation of endogenous neural stem cells after cerebral isehemia is the damage of the collaterals and marrow by toxins. During the acute phase of cerebral isehemia, phlegm, accumulated heat and toxins are present in the brain, which damage the brain marrow and the Du Meridian, and serve as the main factors for inhibiting the proliferation and differentiation of NSCs. The main therapeutic method for this condition is to clear away heat, resolve phlegm and remove obstruction in the collaterals. During the recovery phase with the essence blood in the liver and kidney in deficiency and the remaining toxins, lack of sources of proliferation for NSCs becomes the main pathological factor, and trealment should be given by invigorating the liver and kidney, as well as removing toxins and collateral obstruction. This study has provided new thoughts for the research on the effect of TCM on proliferation and differentiation of endogenous neural stem cell after cerebral isehemia.
