反复自然流产患者绒毛、蜕膜、血清中血管内皮生长因子的表达及绒毛超微结构的研究 被引量:3

摘要 目的:通过对反复自然流产(RSA)患者血清、绒毛、蜕膜中血管内皮生长因子(VEGF)的测定及绒毛超微结构的观察,并与相应正常妇女进行对照,探讨不明原因反复流产患者的病因。方法:采用免疫组化法检测24例RSA患者(实验组)及20例正常女性(对照组)的绒毛和蜕膜组织中VEGF含量,采用扫描电镜观察绒毛组织的超微结构。结果:实验组与对照组平均血清VEGF分别为(106.32±17.26)pg/ml、(88.26±10.15)pg/ml,RSA组高于正常对照组,差异有显著性(P<0.01);RSA组绒毛中VEGF的表达高于对照组,差异有显著性(u=3.243,P<0.01);RSA组与对照组蜕膜中VEGF的表达无显著性差异(u=0.5766,P>0.05)。实验组和对照组血清VEGF水平与绒毛VEGF的表达存在线性相关关系(r1=0.8477,P1<0.05;r2=0.7463,P2<0.05)。实验组和对照组血清VEGF水平与蜕膜VEGF的表达不存在线性相关关系(r1=0.2820,P1>0.05;r2=0.2879,P2>0.05)。结论:①绒毛组织中VEGF含量RSA组高于对照组;②蜕膜组织中VEGF含量RSA组与对照组比较差异无显著性;③扫描电镜观察发现RSA组较对照组绒毛稀疏杂乱,呈颗粒状,微绒毛数量减少,分布欠均匀,密度明显降低,微绒毛增粗、变短、末端圆钝,局部密度增高,反光增强。 Objective:To investigate the possible reasons of RSA and its mechanism through measuring the levels of VEGF in serum,villus and decidua and observed the ultrastructure of the villus in women with RSA and the controls,.Methods:A total of 24 women with RSA(experimental group)and 20 normal women(control)were enrolled in this study.Part of the villus and decidua tissue were observed by scanning electron microscope;The other specimens were studied by immunohistochemically staining for the expression of VEGF.Results:The mean serum VEGF level of the patients [(106.32±17.26)pg/ml] was significantly higher than that of the controls [(88.26±10.15)pg/ml],there were significant differences between the two groups(P 〈0.01).The VEGF level of experimental group in villus was statistically higher than that of the controls(u=3.243,P〈0.01);The difference of VEGF level in decidua showed no statistical significance in the two groups(u=0.5766,P〉0.05);There was correlation between the VEGF level in serum and villus in the two groups(r1=0.847 7,P1〈0.05;r2=0.746 3,P2〈0.05);There was no correlation between the VEGF level in serum and decidua in the two groups(r1=0.282 0,P1〉0.05;r2=0.287 9,P2〉0.05).Conclusion:①The VEGF levels in experimental group in villus are higher than that in the controls;②The difference of the VEGF levels in decidua of the two groups shows no statistical significant;③ Scanning electron microscope find that the microvilli in RSA villus has been decreased significantly,bad-distributed,microvilli is in a mass,dropsy and fractured;it is thicker and shorter than normal villi.
