摘要
目的探讨内分泌激素水平监测对垂体瘤手术远期疗效观察的意义。方法动态观察经显微外科手术治疗的137例垂体瘤患者手术前后血清泌乳素(PRL)、生长激素(GH)、甲状腺功能[T3、T4促甲状腺素(TSH)、血皮质醇(F)和促肾上腺皮质激素(ACTH)]的变化,并进行2年以上的随访和远期疗效评价。结果术后1周内PRL、GH水平明显下降且与以后的PRL、GH水平基本一致,垂体GH瘤患者34例,术后GH降至0.34nmol/L以下者11例,23例GH未降至0.34nmol/L以下,其中13例术后5年内复发。结论术后1周的PRL和GH水平可做为判断手术疗效的指标之一,术后GH降至0.34nmol/L以下可做为判断临床治愈的标准。
Objective To investigate the effect of adenomectomy on endocrine hormones and evaluate its long term therapeutic efficacy.Methods 137 patients with pituitary adenoma were treated by adenomectomy.The dynamic concentrations of serum PRL GH,Cortisone,ACTH and thyroid function (T3,T4,TSH) were monitored before and after microsurgery.All cases had been followed up for more than two years to evaluate the long term therapeutic efficacy.Results PRL and GH levels decreased strikingly within one week after treatment and remained stable afterwards.Among 34 cases of GH-secreting pituitary adenoma,the concentration of GH in 11 cases dropped below 0.34 nmol/L after microsurgery,but 23 cases didn’t,13 patients were recurrent in five years after operation.Conclusion PRL and GH concentrations could be the major indicators to evaluate the effect of surgery; the concentration of GH dropped below 0.34 nmol/L after microsurgery.
