FLAIR与T1WI增强扫描对肺癌脑转移的对比研究

摘要 目的 评价FLAIR与T1WI增强扫描在肺癌脑转移诊断中价值。方法 搜集43例肺癌脑转移患者,对比分析常规T1WI、T2WI、FLAIR及T1WI增强扫描在检出转移瘤灶和明确其边界的敏感性。结果T1WI增强扫描对肺癌脑转移的检出以及肺癌脑转移边界的敏感性明显高于T2WI及T1WI,经统计学分析有明显显著性意义(P〈0.01),T1WI增强扫描较FLAIR以及FLAIR较T2WI有显著性差异(P〈0.05),而T1WI平扫与T2WI无显著性差异(P〉0.05)。结论 对于肺癌脑转移的MRI诊断,T1WI增强扫描的应用明显提高了转移瘤的检出率,尤其是对于无水肿的隐匿性病灶诊断意义更大,可作为常规肺癌脑转移检查的补充序列,而FLAIR可取代常规T2WI。 Objective: To study and compare the sensitivity between FLAIR and Contrast-enhanced T1-weighted imaging sequence in detection of cerebral metastases of lung cancer. Methods: The following was analyzed: the sensitivity of routine T1-weighted images, T2-weighted images, FLAIR images and contrast-enhanced T1-weighted in demonstrating the cerebral metastases of lung cancer and their boundaries in 43 patients. Results: Contrast-enhanced T1-weighted imaging was proved to be the most sensitive in demonstrating the numbers and boundaries of the cerebral metastases of lung cancer among the scanning techniques. FLAIR images were more sensitive than routine T2-we/ghted images in demonstrating the numbers and boundaries of the cerebral metastases of lung cancer. The difference was significant between routine T1-weighted images and T2-weighted images. Conclusion: Contrast-enhanced MRI scan can highly rise metastases of lung cancer check rate, especially on diagnosis of nodropsy metastasis cancer of lung cancer. Contrast-enhanced T1- weighted imaging may be regarded as an useful complementary technique to the conventional imaging sequences. FLAIR sequence can substitude routine T2-weighted images.
