摘要
目的:比较64层螺旋CT冠状动脉成像(64SCTCA)和冠状动脉造影(CAG)对冠心病(CHD)的诊断价值。方法:对86例患者342支血管进行64层螺旋CT冠状动脉成像及冠状动脉造影,并进行对比分析。结果:可用于评估的342支冠状动脉中,64SCTCA共显示病变234支,其中228支得到CAG证实;假阴性14支,64SCTCA诊断冠状动脉病变的敏感性为98.25%,特异性为91.2%,阳性预测值95.7%,阴性预测值96.3%。64SCTCA和CAG2种检查方法在发现冠状动脉病变上差异无统计学意义(P>0.05)。在病变程度上,64SCTCA评估与CAG完全一致者208支,准确性为91.2%。结论:64SCTCA在显示冠状动脉病变时,具有较高的敏感性和特异性,并且对于病变程度的评估也比较准确,对于预测及早期诊断冠心病,有重要的临床应用价值。
Objective: To compare the diagnostic values of 64-slice computed tomography coronary angiography (64SCTCA) and coronary angiography (CAG) for the diagnosis of coronary heart disease. Methods: Eight six patients with 342 vessels were studied by 64-slice computed tomography coronary angiography. The results were compared with coronary artery angiography. Results: Among the 342 coronary artery were evaluated with 234 lesions found, 228 lesions were consisted with CAG, 14 lesions were found by CAG not pick up by 64SCTCA, the sensitivity, specificity, positive predictive value and negative predictive value were 98.25% ,91.2%, 95.7% and 96.3% respectively for the detection of coronary artery disease, Further statistical analysis using 2-related A x^2 test showed no statistically significant difference in the discovery of coronary artery disease between 64 SCTCA and CAG (P 〉0.05). There was concordance in assessing lesion severity in 208 lesions, and accuracy approaches 91.2%. Conclusion: 64 SCTCA has excellent sensitivity and specificity in detecting coronary artery disease, and also can give fairly accurate assessment stenosis severity in most patients as compare to CAG, there are important clinical value in forecasting and early diagnosis of coronary heart disease.
