摘要 目的评价3岁以内婴幼儿动脉导管未闭应用蘑菇伞封堵器介入治疗的临床效果。方法全组21例,年龄9个月~3岁,平均(2.2±0.8)岁,体重6.0~13.2kg,平均(12.6±2.5)kg。主动脉侧位造影确定动脉导管的形态和导管最窄处直径,选择合适封堵器。经静脉途径置入封堵器。术后24h、1个月、3个月、1年、2年及3年复查胸部X线平片及超声心动图和心电图观察封堵器的位置、残余分流情况、肺动脉压及心脏大小。结果21例患儿植入封堵器均获成功,技术成功率为100%。造影测定PDA最窄处直径2.5~10.9(5.8±2.6)mm。应用直径4~18mm大小的PDA封堵器进行封堵。随访无严重并发症。结论应用蘑菇伞封堵器治疗婴幼儿期动脉导管未闭具有操作简便、安全有效、技术成功率高及封堵效果好等优点。 Objectives To evaluate the therapeutic effect of transcatheter closure in treating infants with patent ductus arteriosus using occluder device. Methods Twenty-one (21) patients with an average age of 2.2 ± 0.76 years (from 9 months to 3 years) and weight of 12.6 ± 2.52 (6.0 to 13.2) kg, were included. A lateral view aortogram was used for morphological determination of the minimal diameter of the ductus to facilitate the choice of suitable device. Occluder was implanted using the anterograde venous approach. Chest X-ray and echocardiogram were given during followup at 24 hours and 1, 3, 6 months, and 1,2, 3 years after implantation. Results The devices were successfully implanted in all patients. The minimal diameter of the ducts showed angiographically from 2.5-10.9 mm, and the size of occluder used was from 6 mm to 18 mm. No obvious complications were observed during the follow-up period. Conclusions Transcatheter closure of PDA with occluder device is a safe and effective method for the treatment of PDA in infants and young children.
