摘要
目的:通过小儿先天性心脏病封堵治疗的临床分析,对封堵治疗的疗效进行初步评估并探讨适合小儿先天性心脏病封堵治疗较理想的麻醉方法。方法:60例先天性心脏病患儿动脉导管未闭(PDA)33例,房间隔缺损(ASD)18例,室间隔缺损(VSD)9例,患者随机分为两组,麻醉开始均静脉注射氯胺酮,一组(Y组)以微量注射泵持续泵入异丙酚维持,另一组(K组)先静脉注射力月息后以微量注射泵持续输注氯胺酮维持,术中观察患儿肢体活动、眼脸反射、呼吸、HR、Bp、ECG、SpO2变化,调整药物用量。结果:封堵治疗效果满意,成功率98.3%;异丙酚组患儿术中安静、经过平稳,可较好地耐受手术,且呼吸循环影响轻微,诱导及苏醒迅速,无明显副作用及并发症。结论:应用导管封堵治疗小儿先天性心脏病是有效的非手术治疗方法;异丙酚静脉麻醉是先心病患儿封堵治疗较理想的麻醉方式。
Objective: To examine the safer and more effective anesthesia method through clinical analysis of intervenfional therapy of children congenital heart diseases.Methods: Sixty chidren of congenital heart diseases (33 cases were PDA. 18 cases were ASD, 9 cases were VSD) were randomly divided into two groups: Group Ⅰ (group Y,n= 30)in which anesthesia was induced with ketamine and was supplemented with propofol by microinjection confinual pump; in group Ⅱ (group K, n= 30)anesthesia was also induced with ketamine, but was supplemented with midazolam and ketamine by microinjection confinual pump. We adjusted the amount of drug by observing the changes of body activity, eyelid reflection, breath, HR, BP, ECG, and SpO2. Results: The occlusion effect of interventional therapy was sastisfactory.The success rate was 98.3%. The two groups were comparable with respect to above data. Compared with group Ⅱ , the children in group Ⅰ could endure the operation more peacefully and steadily, the changes of respiratory and circulation systems were more slight and without obviously side effects. Conclusion: The interventional therapy is an efficient non-operation method for congenital heart diseases in children with a high success rate and a good occlusion effect. Propofol anesthesia by intravenous is suitable for them.
