摘要
目的观察碘化油乳化As2O3经肝动脉栓塞实验兔肝种植瘤灶的疗效。方法40只VX-2兔肝模型，行肝动脉插管后随机分为A、B、C、D四组即碘化油乳化As2O3组、生理盐水组、碘化油组和As2O3，组，术前、术后7d分别采耳静脉血测前白蛋白（PA）、白细胞计数，2周后处死实验兔取瘤灶测其体积，行病理学检查。结果碘化油乳化As2O3，经肝动脉栓塞组无腹水及转移，在肿瘤体积变化与各对照组相比较差异有统计学意义（p〈0．001）；病理示A组种植结节呈坏死无结构样表现，坏死灶周围见假包膜之层状纤维细胞，见淋巴细胞、浆细胞，只有散在活肿瘤细胞。而各对照组瘤结节中心呈程度不一坏死无结构样表现，而其周缘区少许淋巴细胞、浆细胞，仍见活体瘤组织，尤以B组为甚，D组次之。碘化油乳化As2O3组用药3d时前白蛋白比治疗前稍降低，但其差异无统计学意义（p=0.825），而7d时其前白蛋反而高于治疗前及用药3d值，其差异有统计意义（P〈0.001），其各时间值与单用碘油及单用As2O3比较差异无统计学意义（P〉0.05），另外各组不同时间白细胞变化差异无统计学意义（P〉0．05）。结论碘化油乳化As2O3经肝动脉栓塞治疗实验兔肝种植瘤灶是安全有效的。其疗效优于对照组。
Objective To observe the effect of hepatic artery embolization of rabbit implanted VX-2 liver carcinoma by As2O3 lipiodol emulsion. Methods 40 rabbit implanted VX-2 liver carcinoma model were randomly divided into A, B, C and D Group after hepatic artery intubation： As2O3 lipiodol emulsion, NS, iodized oil and As2O3 Group. Prealbumin（PA） and white blood cell count by ear vein blood were tested before and after 7 days of operation, Two weeks later rabbits were killed, tumor volume was measured and histopathological examination was done. Results No ascites or transfer was found in As2O3 lipiodol emulsion group, its changes in tumor volume had a significant difference compare to which of the control group（p〈0.001）.Pathology showed that planted tuberous appeared to be necrosis of no structure with layered fibroblasts circle around in A group, lymphocytes, plasma cells and scattered living tumor cells also can be seen too. The degree of necrosis was lighter in tumor center in control groups, with living tumor tissues（which was found especially in group B, and followed in D）, few lymphocytes and plasma cells around. Prealbumin（PA）lower slightly than before After 3 days treatment in As2O3 lipiodol emulsion Group, but the difference was not statistically significant（p=0. 65）, after 7day＇ s treatment, its value is higher than that of the treatment before and after 3 days, which differences are statistically significant（p〈0.001）, There is no statistically significant difference compared to its time value and single-use iodized oil and As2O3 （p〉0.05）.In addition, there was no significant difference compared to the changes in leukocyte of different time in each groups （p〉0. 05）. Conclusion It was safe and effective to use As2O3 lipiodol emulsion embolize hepatic artery of rabbit implanted VX-2 liver carcinoma, lts effect is better than control group.
