嗅鞘细胞移植部位与剂量的选择对陈旧性脊髓损伤患者60例神经恢复的影响 被引量:2

摘要 目的:探讨嗅鞘细胞(Olfactory esheating cells OECs)移植部位与剂量治疗陈旧性脊髓损伤疗效的影响。方法:应用我院实验室由人胚胎的颅脑嗅球提取、纯化,培养1-2周的OECs,常规脊椎后入路切开硬脊膜借助手术显微镜将50 ul OECs悬液单点移植于受损脊髓上段与正常脊髓交界处(共30例A组)。用同样方法将50 ul×2(最多可达50 ul×4)OECs分多点移植于损伤脊髓上下端,即损伤脊髓与正常脊髓的交界靠近正常一侧(共30例B组)。结果:在我院自愿接受OECs移植的106例陈旧性脊髓损伤患者在术后2-4周内均应用美国脊柱脊髓损伤学会(ASIA)标准进行评价。患者在感觉、运动、大小便及植物神经功能方面均有不同程度的改善。其中B组病人较A组病人恢复更为明显。结论:手术将50 ul×2(最多可达50 ul×4)OECs分多点移植于损伤脊髓上下端优于将50 ul OECs单点移植于损伤脊髓上端的治疗方法。 Objective:To explore the effect of sites and dosage in olfactory ensheathing cells (OECs) transplantation on curing dated spinal cord injury. Methods:OECs were purified from human olfactory bulb and cultured for 1 or 2 weeks in the laboratory of our hospital. The spinal dural mater was conventionally cut through the dorsal approach of vertebrae , 50ul OECs suspension were transplanted under microscope to one sites in the border of the superior part of the injured spinal cord and the normal spinal cord (30 cases in Group A). To transplant 50ul×20ECs (at most 50ul×4) to several sites in the either side of the injured spinal cord (the border of injured spinal cord and normal spinal cord that approach to the normal spinal cord) with the same method (30 cases in Group B). Results : To evaluate the 106 cases of injured spinal cord who accepted the OECs transplantation in our hospital in 2-4 weeks after the operation with ASIA. The cases have different improvements in sense, movement, defecating, urinating and function of vegetative nerve. The improvements of the cases in Group B are more obvious than Group A. Conclusions : Transplanting 50ul×20ECs ( at most 50ul×4) to several sites in the either side of the injured spinal cord is superior to the therapy that transplanting 50 ul OECs to one sites in the superior part of the injured spinal cord.
