摘要
目的:胚胎脊髓移植治疗脊髓损伤的实验研究已进行多年,效果并不理想,带蒂大网膜移植可建立及时有效的血供是肯定的,对脊髓损伤的疗效尚无定论。为此探讨采用犬胚胎脊髓和大网膜联合移植治疗脊髓损伤的效果。方法:选用西北犬24只,雌雄不拘,按随机数字分成4组:单纯损伤组;大网膜移植组;单纯胚髓细胞移植组;胚胎脊髓和大网膜联合移植组。致伤前及治疗后2个月行脊髓体感诱发电位,动作诱发电位检查,观察行为变化,并行电镜及苏木精-伊红染色检查。结果:伤后2个月胚胎脊髓和大网膜联合移植组左后肢脊髓体感诱发电位潜伏期71.65±3.33)ms与各组间对比差异具有显著性意义F=3.891,((P=0.037),脊髓体感诱发电位波幅1.04±0.11)μV与各组间对比差异(具有显著性意义(F=5.516,P=0.013),左后肢动作诱发电位潜伏期(6.28±1.45)ms与各组间对比差异具有显著性意义(F=6.49,P=0.008),行为学有恢复;胚髓细胞在术后2个月可存活。结论:胚胎脊髓与大网膜联合移植治疗脊髓损伤动物从行为学和诱发电位均优于其他组,但尚需进一步研究以确定其功能。
AIM:The experimental study on the fetal spinal cord transplantation for treatm ent of the spinal cord injury (SCI)has been lasted for many years and with a poo r result.The greater omental transposition with vascular pedicle can establish t he blood supply timely and effectively,the final conclusion has not yet been rea ched on the therapeutic effectiveness for SCI.To observe the clinical efficacy o f the fetal spinal cord combined with greater omental transposition for treatmen t of the SCI. METHODS:Twenty four northwest adult dogs of either gender,were involved in th is experimental study and randomly divided into 4 groups: simple injury group;gr eater omental transposition group;fetal spinal cord transplantation group;fetal spinal cord transplantation combined with greater omental transposition group.So matosensory evoked potential(SEP) and motor evoked potential(MEP) of the spinal cord were measured before injury and 2 months after the treatment to observe the changes of the animals' behavior.Observation under the electron microscope and hematoxylin eosin staining were performed. RESULTS:There was a significant difference in the latent period of the left hi ndleg's SEP of the dogs between fetal spinal cord transplantion and greater omen tal transposition group [(71.65±3.33)ms] and other groups (F=3.891,P=0.037), a significant difference in the wave amplitude between fetal spinal cord transpl antion and greater omental transposition group and other groups [(1.04±0.11) μV](F=5.516,P=0.013).In addition,there was a significant difference in the late nt period of the left hindleg's MEP of the dogs between group fetal spinal cord transplantion and greater omental transposition group and other groups [(6.28± 1.45)ms](F=6.49,P=0.008).Recovery of the behavior was noted.The embryonic cells survived two months after the transplantation. CONCLUSION:Transplantation of the fetal spinal cord and transposition greater omentum for treatment of the injured spinal cord in dogs can gain a better outco me than the
