54例神经导航引导下星形细胞肿瘤切除术的经验 被引量:2

摘要 背景与目的:神经导航外科是微侵袭神经外科的重要组成部分,本研究评价神经导航在颅内星形细胞肿瘤显微手术中的应用价值。方法:回顾性分析哈尔滨医科大学肿瘤医院神经外科于2004年9月至2007年4月应用神经导航系引导进行颅内星形细胞肿瘤切除术的54例病例资料。结果:用神经导航系统进行星形细胞肿瘤及其周围解剖结构定位准确,平均注册误差(1.42±0.46)mm。术后近期复查,CT或MRI证实肿瘤全切36例(78%),次全切除10例,大部切除8例。所有肿瘤全切患者术后增强CT或MRI检查未发现肿瘤残留病灶。结论:神经导航定位精确,有助于手术计划的精确实施。实时导航颅内星形细胞肿瘤显微手术,有助于达到微侵袭效果和提高肿瘤全切除率。 BACKGROUND & OBJECTIVE:To assess the clinical value of neuronavigation in the resection of intracranial astrocytomas. METHODS: From September 2004to April 2007, 54 patients with intracranial astroeytomas were operated with the assistance ofneuronavigation system. The diameter of tumors ranged from 2.8cm to 7.0 cm, with a mean diameter of 6.5cm. The operations were guided by neuronavigation system. RESULTS: The tumors and the surrounding structures were located accurately with mean registration error 1.42±0.46mm. Among 54 cases, total removal was achieved in 36 cases, subtotal removal in 10 cases and partial removal in 8 cases. CONCLUSION: The neuronavigation system is accurate in real-time orientation of intracranial astrocytomas microsurgery. It improve the resection of intracranial astrocytomas with micro-invasion.
