摘要
目的:本研究采用腹主动脉缩窄法建立兔高血压左室肥厚模型,评价高血压左室肥厚对左房直径(LAD)与心房利钠肽水平的影响及后两者的相关关系。方法:大耳白兔14只随机分为假手术组(n=6),左室肥厚组(n=8)。手术后8周进行超声心动图检查评价左室肥厚程度,LAD和左室射血分数(LVEF);然后应用多导电生理记录仪进行血流动力学测定,下腔静脉取血测定血浆心房利钠肽(ANP)水平。结果:术后8周超声心动图结果显示,左室肥厚组左房直径较假手术组明显扩大(7.61±1.23vs5.06±1.12mm,P<0.01);左室间隔舒张末期厚度(3.13±1.08vs2.03±0.32mm,P<0.05)和左室后壁舒张末期厚度(2.69±0.66vs2.05±0.23mm,P<0.05)均显著增加。血流动力学结果显示与假手术组比较,左室肥厚组主动脉收缩压、舒张压、左室压力最大上升速率和下降速率均显著增加。与假手术组相比,左室肥厚组血浆ANP水平明显升高(1.04±0.65vs0.44±0.05ng/L,P<0.05)。相关性分析显示LAD与血浆ANP水平存在显著正相关(r=0.665,P<0.01)。结论:高血压左室肥厚可导致左房直径及血浆ANP水平明显增加,且ANP水平可以间接反映左房扩大的程度。
Objective: A rabbit model of left ventricular hypertrophy(LVH) was made by abdominal aortic coarctation. To evaluate the effects of LVH on left atrial diameter(LAD) and the atrial natriuretic peptide (ANP) and the interrelationship between LAD and ANP. Methods: Fourteen rabbits were randomly divided into two groups: LVH group (n= 8) which was induced by abdominal aortic constriction and sham-operated group (n = 6). Echocardiographic parameters such as LAD and left ventricular ejection factor (LVEF) and hemodynamic measurements were assessed 8 weeks after operation. Blood sample were also obtained to measure ANP level. Results: Eight weeks after operation, Left atrial diameter(7.61±1.23 vs 5 06±1. 12mm, P〈0. 01), interventricular septum diastolic thickness (3.13±1.08 vs 2.03±0.32mm, P〈0.05) and left ventricular posterior wall diastolic thickness (2. 69±0. 66 vs 2.05±0. 23 mm, P〈0. 05) in LVH rabbits were significantly larger than those in sham rabbits. Compared with Sham group, SBP.DBP.LV+dp/dtmax and-dp/dtmax in LVH group were also increased significantly. ANP level elevated significantly in LVH group than that in sham group (1. 04±0. 65 vs 0. 44±0. 05ng/L, P〈0. 05). There was also significant correlation between LAD and ANP (r= 0. 665, P〈0.01). Conculsion; Left ventricular hypertrophy can induce significant elevation of LAD and ANP, and ANP level can reflect the degree of left atrial dilation indirectly.
