摘要 目的探讨CTA减影技术对于脑静脉系统病变的诊断价值。方法采用64层螺旋CT行头部或头颈部CT血管造影(CTA),回顾性分析我科20例确诊(经DSA或和手术证实)脑静脉系统病变患者CTA减影图像质量,并比较常规CTA与减影CTA显示病变的能力。结果经CTA诊断颅内静脉窦血栓患者4例,静脉畸形患者8例,肿瘤累及静脉窦患者5例,外伤累及静脉窦患者2例,颈内动脉海绵窦瘘患者1例。20例患者减影CTA病变显示率为95%,优于常规CTA;90%病例减影图像质量优良,血管清晰显示,少数因头部轻微运动,颅骨及颅底骨未能完全减影,通过结合各种三维重建方法(VR、MIP、MPR),不影响诊断。结论CTA减影技术诊断脑静脉系统病变可行、有效。 Objective To evaluate the diagnostic value of subtraction CT angiography in cerebral venous diseases. Methods The image quality of subtraction of 20 cases of cerebral venous diseases, confirmed by DSA and/or surgically, by head or head-and-neck scanning with 64-row CT was analyzed, and the ability of conventional CT angiography and subtraction CT angiography to display focus was compared. Results The focus displaying rate by subtraction CT angiography was 100%, including 4 cases of cerebral venous sinus thrombosis, 8 venous malformation, 5 tumor-involved sinus diseases, 2 trauma-involved sinus diseases and 1 carotid-cavernous fistula, higher than conventional CT angiography. Image quality of subtraction was good and vessels were displayed clearly for 90% cases. But in a few cases, the cranium and skull base could not be completely removed due to slight motion of head, which did not influence the diagnosis after some postproeessing by volume rendering (VR), maximum intensity projection (MIP) and multiplanar reconstruction (MPR). Conclusion Subtraction CT angiography is reliable, effective and worthy of wide clinical application in diagnosis of cerebral venous diseases.
