摘要 目的:探讨用脑血管对比剂达峰时间来推断头颈容积CT数字减影血管造影(Volume CT Digitial Subtraction Angiography,VCTDSA)的扫描延迟时间的可行性方法:回顾性分析了108例脑血管和41例头颈血管VCTDSA的对比剂团注测试达峰时间,测量左侧大脑中动脉M1段近端、上矢状窦、C4椎体水平左侧颈总动脉和颈内静脉达峰时间及达峰强化CT值。结果:颈总动脉和大脑中动脉的对比剂达峰时间相差0.22s,差异无统计学意义(t=0.48,P>0.05);颈内静脉和上矢状窦的对比剂达峰时间相差1.31s,大脑中动脉和上矢状窦的对比剂达峰时间相差6.06s,颈总动脉和颈内静脉的对比剂达峰时间相差7.59s对比剂达峰时C4椎体水平左侧颈总动脉和颈内静脉强化程度高于左侧大脑中动脉M1段近端和上矢状窦。结论:脑及头颈VCTDSA的对比剂团注测试得到的脑及颈血管对比剂达峰时间无明显差异,可以用脑血管对比剂达峰时间求推断头颈VCTDSA的扫描延迟时间。 Objective:To discuss the feasibility of the scan-delay-time in cerebral and cervix with Volume CT Digitial Subtraction Angiography(VCTDSA) deducing by the time-to-peak(TP) in dynamic scan with CT perfusion imaging. Method: Analysis was performed in 149 patients who had examined the cerebral and cervical vessel with VCTDSA,while 108 cases examining Cerebral Vessels,the other 41 examining Cephalic and Cervical Vessel. They were all measured with TP and enhancement value of CT in the M1 segment of left middle cerebral artery(LMCA),superior sagittal sinus(SSS),lefi common carotid artery(LCCA) adjacent to the fourth cervical vertebrae(C4) and internal jugular vein(IJV). Results:The discrepancy of TP between CCA and MCA was 0.22 second,without statistically different,P〉0.05. While between IJV and SSS it was 1.31 second, between MCA and SSS 6.06 second,between CCA and IJV 7.59 second. At the same time of TP,the left common carotid artery adjacent to the fourth cervical vertebrae and IJV enhanced significantly more than the M1 segment of left middle cerebral artery and SSS. Conclusion:The TP in Cerebral Vessels and Cephalic and Cervical Vessel was not obviously difference testing by contrast agent mass injection method. So we can deduce the scan-delay-time in cerebral and cervix with VCTDSA by the TP with CT perfusion dynamic scan.
