摘要
目的探讨乌司他丁(UTI)对大鼠脓毒症急性肺损伤外周血淋巴细胞凋亡率及血浆细胞因子TNF-α、IL-6水平的影响。方法盲肠结扎穿孔术(CIP)制备脓毒症急性肺损伤模型,将SD大鼠随机等分成假手术组(Sham)、脓毒症急性肺损伤组(ALI组)、左旋氧氟沙星治疗组(LEV组)、乌司他丁治疗组(UTI组),每组再等分4个亚组,每亚组7只。检测外周血淋巴细胞凋亡率及血浆TNF-α、IL-6水平的变化,并观察肺组织的病理形态学改变。数据以均数±标准差(x^-±s)表示,采用SPSS13.0统计软件处理,多组均数间比较进行方差齐性检验和单因素方差分析(one-way ANOVA),以P〈0.05为差异有统计学意义。结果脓毒症急性肺损伤时外周血淋巴细胞凋亡率升高,UTI治疗组的淋巴细胞凋亡率较脓毒症急性肺损伤组及左旋氧氟沙星治疗组明显降低(P〈0.05);血浆TNF-α和IL-6水平分别于CIP术后3h和6h时出现升高,UTI治疗组与脓毒症急性肺损伤组及左旋氧氟沙星治疗组均明显降低(P〈0.05);UTI治疗组大鼠肺损伤程度明显减轻。结论乌司他丁能显著抑制脓毒症肺损伤大鼠外周血淋巴细胞凋亡及血浆TNF-α和IL-6表达水平,能显著减轻肺损伤的病理损害,对大鼠脓毒症急性肺损伤具有明显的保护作用。
Objective To observe effects of UTI on apoptosis rate of lymphocyte and plasma level of TNF- α, IL-6 on acute lung injury (ALl) induced by sepsis, and to investigate the mechanism of UTI for protecting the lung against acute lung injury induced by sepsis. Method The ALl rats were established by cecal ligation and puncture (CLP), Totally 112 SD rats were randomized into four groups: sham operated group (sham), acute lung injury group (ALI) , UTI treated group (UTI) and levotloxacin treated group (LEV). Each group was subdivided into 4 subgroups: 3-hour group (T3), 6-hour group (T6), 12-hour group (T12) and 24-hour group (324). Lymphocytie apoptosis in peripheral blood and changes of plasma TNF-α, IL-6 were detected. Histological examination of the lungs was also performed. The data were expressed as (x^-± s ), and analyzed with chi-square test and one-way ANOVA using SPSS 13.0. A P value less than 0.05 indicated statistical significance. Results The apoptusis rate of lymphocyte in ALl group increased obviously, and the apoptosis rate of UTI treated group was lower than that of ALl and LEV group ( P 〈0.05). The plasma level of TNF-α at 3 hours in UTI gToup was lower than that in ALl group (P 〈0.01). The plasma level of IL-6 in ALl group increased at 6 hours after CLP. The plasma level of IL-6 at 6 hours after CLP in UTI group was lower than that in ALl and LEV group ( P 〈 0.05). The histological changes of lung injury in UTI group improved significantly compared with ALl and LEV group. Conclusions UTI could inhibit apoptosis of lymphocyte, expression of TNF-α and IL-6. UTI could improve histomorphologyof lung injury. UTI had protective effects on the lungs against acute lung injury induced by sepsis in rats.
