摘要
目的探讨血浆同型半胱氨酸(HCY)水平与冠心病(CHD)传统危险因素的关系。方法选择123例CHD患者(CHD组)及排除CHD者101例作为对照组,用荧光偏振免疫分析法测定血浆HCY浓度,同时测定血清总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL—C),并通过问卷调查了解所有入选者高血压病(EH)、糖尿病(DM)史、吸烟史、相关家族史(FH)等。结果CHD组血浆HCY平均浓度(15.04±7.58)μmol/L显著高于对照组(10.08±3.11/μmol/L,P〈0.01),高同型半胱氨酸血症(HHCY)的发生率明显高于对照组(P〈0.01),CHD组血浆HCY水平与年龄、性别、血脂各指标、高血压史、糖尿病史、吸烟史及FH无相关关系;经多因素Logistic回归分析显示HCY是CHD的独立危险因素,其OR值为1.238(95%的可信区间为1.109-1.381,P〈0.01)。结论血浆HCY水平和CHD传统危险因素之间无相关关系,HHCY可能是CHD发生的一个独立危险因素,血浆HCY水平检测,有助CHD的诊断。
Objective To explore the correlation between plasma homocysteine(HCY) concentrations and some established risk factors of coronary heart disease(CHD). Methods Plasma HCY concentrations were measured by fluorescence polarization immunoassays ( FPIA ). Serum triglyceride ( TG ), total cholesterol (TC), high density lipoprotein- cholesterol( HDL-C) and low density lipoprotein- cholesterol (LDL-C)of 123 CHD patients and 101 controls were measured respectively. Information in medical histories including essential hypertension(EH), diabetes mellitus(DM), related family history(FH)and smoking status were obtained from all participants via a questionnaire. Results (1) Plasma HCY concentrations of patients with CHD were higher than that in controls (P 〈 0.01). There was significantly higher incidence of HHCY in CHD group than those in control group(36. 59% vs 9.90%, P 〈 0.01). (2) There was no correlation between Ievels of HCY and age, sex, TG, TC, I-IDLC or LDL-C in the CHD group. There was no discrepancy in Ievels of HCY between cases with different history of EH, DM, FH and smoking status. (3) Based on multivariate logistic regression analysis, the odds ratio(OR)of HCY to CHD was 1.238(95% confidence interval, 1. 109-1.381, P 〈 0.01). Conclusion There is no correlation between plasma homocysteine and established risk factors of coronary heart discase. HCY is an independent risk factor of CHD. Detection of the levels of Plasma HCY can be a helpful marker for diagnosing CHD.
