摘要 学术背景:移植的脂肪颗粒坏死后往往引起纤维囊性化和假性囊肿,此外在治疗中还存在液化、感染等并发症,阻碍了脂肪组织作为软组织填充物在临床上的应用。近年来利用干细胞体内诱导成脂肪组织备受关注,此项技术若能成功应用于临床将会解决传统移植成熟脂肪细胞的各种并发症。目的:深入认识脂肪干细胞在脂肪组织工程中的应用进展。检索策略:由该论文的研究人员应用计算机检索Pubmed数据库1998-02/2007-02的相关文献,检索词"mesenchymal stem cell,adipose tissue-derived stem cell/adipose tissue-derived stromal cell,tissue engineering,review",并限定文章语言种类为English。共检索到180篇文献,对资料进行初审,纳入标准:①文章所述内容应与脂肪干细胞及其在脂肪组织工程中的应用密切相关。②同一领域选择近期发表或在权威杂志上发表的文章。排除标准:①重复性研究。②Meta分析。文献评价:文献的来源主要是通过对脂肪干细胞在脂肪组织工程中的应用方面内容进行汇总分析。所选用的30篇文献中,6篇为综述,其余均为临床或基础实验研究。资料综合:①脂肪干细胞在形态上与间充质干细胞相似,表达CD44证明其来源于干细胞,可在无血清条件下进行体外培养。在表型上,表达CD29,CD44,CD49,CD71,CD90,CD105,SH2,SH3,不表达CD31,CD34,CD45,CD106。②脂肪干细胞向脂肪分化经历以下过程:多能干细胞→脂肪母细胞→前脂肪细胞→不成熟脂肪细胞→成熟脂肪细胞。过氧化物酶增殖物活化受体和C/EBP家族是控制脂肪细胞分化的中心环节,二者共同协调作用导致脂肪酸连接蛋白、磷酸烯醇式丙酮酸激酶、葡萄糖转运体4和胰岛素受体等基因的表达,这些基因的表达正是脂肪细胞成熟的标志。③种子细胞最终移入体内还需要合适的载体,这种载体必须具有生物相容性,并可生物降解,易于生长因子等促细� BACKGROUND: The necrosis of implanted fat granules results in cystic fibrosis and pseudocyst, and there are many complications such as liquefaction, infection and so on. These inhibit the application of fat tissues as soft tissue filling in clinic. Recently, studies on stem cells inducing adipose tissue in vivo are paid more attention. If this technique is successful, various complications in traditional transplantation with mature adipocytes can be prevented. OBJECTIVE: To summarize the application of adipose tissue-derived stem cells in adipose tissue engineering. RETRIEVAL STRATEGY: The computer-based online search of Medline was undertaken to identify related articles published between February 1998 and February 2007 with the key wards"mesenchymal stem cell, adipose tissue-derived stem cell/adipose tissue-derived stromal cells, tissue engineering, review" in English. 180 retrieved articles were checked primarily. Inclusive criteria: (1)articles about the isolation, purification, genotype and application of adipose tissue-derived stem cells in adipose tissue engineering were screened out; (2)in the same field, articles recently published or in authoritative journals were selected. Exclusive criteria: (1)the repeated research, (2)Meta analysis were eliminated. LITERATURE EVALUATION: Articles mainly analyze the application of adipose tissue-derived stem cells in adipose tissue engineering. Of the 30 included articles, 6 were review and others were basic or clinical researches. DATA SYNTHESIS: (1)Adipose tissue-derived stem cells and mesenchymal stem cells are morphologically alike. Both of them can express CD44. Adipose tissue-derived stem cells can be cultured in vitro under the serum-free medium, Adipose tissue-derived stem cellsarepositiveforCD29, CD44, CD49, CD71, CD90, CDI05, SH2, SH3, and negative for CD31, CD34, CD45, CDI06. (2)Adipose tissue-derived stem cells are able to differentiate into adipocytes, as follows, multipotential stem cells→adipose blast cells�
