乳腺癌术后应用腹壁下动脉穿支皮瓣游离移植一期乳房再造的临床研究 被引量:2

摘要 【目的】探讨乳腺癌术后即刻应用腹壁下动脉穿支(DIEP)皮瓣游离移植行乳房再造的方法与疗效。【方法】2005年12月至2007年05月,对10例乳腺癌(Ⅱ期2例,Ⅲ期6例,Ⅳ期2例)患者行根治术或改良根治术,应用双蒂D IEP皮瓣Ⅰ期再造乳房。所有患者术后均行常规化疗及放疗。【结果】全部皮瓣均成活,无并发症。随访半年以上,未见肿瘤复发和转移,未见腹壁疝和腹壁膨出发生。再造乳房外形可,与健侧对称性可,弹性好,患者自我满意度高,对生活充满信心。【结论】乳腺癌术后Ⅰ期应用DIEP游离移植再造乳房,具有皮瓣血运良好、再造乳房外形满意、腹部供区并发症少,可同时达到腹壁整形的目的等优点,是乳腺癌术后乳房再造的一种理想方法之一。放化疗不影响皮瓣的成活。 [Objective] To evaluate the clinical method and therapeutic effect of breast reconstruction through free grafting of deep inferior epigastric perforator (DIEP) flap after mastectomy . [Methods] Ten cases with breast cancer (2 in Stage Ⅱ ,6 in Stage Ⅲ and 2 in Stage Ⅳ ) enrolled from Dec. 2005 to May 2007 underwent mammaplasty through DIEP flap after radical mastectomy or modified radical mastectomy. All of them received routine chemotherapy or radiotherapy after operation. [Results]All flaps in 10 cases survived without complications. After six months follow up, the results showed no recurrence, metastasis, lateral ventral syn- drome and bulge of abdominal wall. All the reconstructive breasts appeared good outline, symmetry and elasticity compared with their uninjured sides. Patients had well satisfaction and confidence. [Conclusion] Mammaplasty through DIEP flap has merits such as satisfactory outline, few complications in belly donor site and fine blood supply, and meanwhile gets the effect of abdominoplasty. It is an ideal method of mammaplasty. Both chemotherapy and radiotherapy have no effect on flap survival.
