Evaluation of left ventricular torsion in patients with dilated cardiomyopathy by speckle tracking echocardiography

摘要 目的应用超声斑点追踪成像技术评价扩张型心肌病(DCM)患者左心扭转及左心扭转与左心收缩功能的关系。方法获取31例DCM患者与30例正常人的标准短轴左心室二尖瓣水平、乳头肌水平、心尖水平的图像,应用超声斑点追踪技术测量短轴各个水平收缩期整体峰值圆周应变,并测量二尖瓣水平、心尖水平心室的旋转角度。比较DCM组及正常组的收缩期整体峰值圆周应变和旋转角度、达到旋转角度峰值时间百分比,以及最大圆周应变、扭转与左心室射血分数的关系。结果与正常人相比,DCM患者收缩期短轴左心室二尖瓣水平、乳头肌水平、心尖水平整体最大圆周应变及旋转角度明显低于正常组(P〈0.01),心尖部达峰值角度时间较正常人提前。且左心室短轴各水平总体收缩期圆周应变峰值均与患者的左室射血分数呈正相关,左心室扭转与患者的左室射血分数呈正相关。结论DCM患者的左心旋转收缩功能显著受损,超声斑点追踪技术可以准确反映左心室收缩功能,对DCM左心收缩功能的判断具有重要价值。 Objective To evaluate the systolic function and torsion of left ventricle in patients with dilated cardiomyopathy by using speckle tracking echocardiography. Methods Two-dimensional echocardiographic images were acquired from the left ventricular short-axis views at the mitral annulus,and apical levels of the left ventricle in 31 patients with dilated cardiomyopathy and 30 healthy subjects. Peak torsion was defined as the maximum difference in rotation angle between the base and apax. Time to peak torsion was expressed as % systole by dividing the time by a total systolic time. The peak systolic circumferential global strain and rotation were measured in the left ventricular short-axis views using 2D strain software. Results The peak systolic circumferential global strain and the peak torsion in dilated cardiomyopathy were statistically significantly lower than in controls( P〈0. 01), and timing of peak torsion was earlier in apical level in dilated cardiomyopathy. Torsion measurement by speckle tracking echocardiography showed good correlation and agreement with left ventricular ejection fraction. Conclusions Left ventricular systolic torsion in patients with dilated cardiomyopathy was significantly impaired. Torsion measurement by speckle tracking echocardiography is a new echocardiographic approach for quantification of left ventricular systolic function.
