应用多层螺旋CT曲面重建技术显示阑尾全貌 被引量:2

摘要 目的:探讨多层螺旋CT(MSCT)曲面重建技术(CPR)在阑尾全貌显示及其径线测量中的应用价值。方法:对129例正常人行MSCT检查。在重建图像上应用单层CPR追踪阑尾,得到CPR像。应用组内相关系数(ICC)评价CPR阑尾径线测量的可重复性。应用CPR与多平面重建(MPR)图像对照及铜线模拟阑尾的方法评价CPR像上阑尾径线测量的真实性。结果:Kappa检验结果显示不同观察者之间阑尾的检出率及CPR图像质量评价具有极佳的一致性。对于CPR显示阑尾全貌,观察者间组内相关系数(ICC)为0.98;观察者内部ICC为0.99,均具有很好的可重复性。阑尾中部外径测量及铜线模拟阑尾实验结果显示CPR测量具有高度的真实性。结论:CPR可以很好地显示阑尾全貌,并可以用于阑尾径线测量。 Objective: To evaluate the value of curved plannar reformation (CPR) of multislice CT (MSCT)in full length displaying and measurement of appendix vermiform. Methods: 129 healthy adults were examined with MSCT. The reconstructed data were transfered to the workstation. Three observers who were familiar with CT anatomy of appendix veriform looked for the appendix. One observer traced the appendix in MPR images with single-slice CPR and got CPR images.Intraclass correlation coefficient (ICC) were used to estimate the reliability of measurement and frequency of visulaization of appendixes. The validity of CPR for appendix was evaluated with copper-line simualted appendix experiment and comparison of the CPR and MPR images. Results: The frequency of visualization of appendix of three obsvers was 93.02%, 92.25% ,92.25% respectively. 87.6% of appendixes were found by all the three observers.The kappa test showed excellent reliability among the three observers. The interrater ICC for quality of CPR images was 0.98,the intrarater ICC for that was 0.99 (P〈0.05).The outer diameters of appendixes was (6.1±2.2) mm,(6.0±2.0) mm separately in CPR and MPR image. No significant difference was found between those groups (P 〉0.05). The mean length of cooper-lines measured with vernier caliper before and after bending was (7.47±1.43)mm. The mean length of cooper-lines in CPR was (7.44±1.39) mm. There was no significant between those two groups (P〉0.05).Conclusion: CPR is excellent for showing the appendix,and can be used in the measurement of appendix.
