天然牙-种植体联合支持式固定义齿的临床观察 被引量:4

摘要 目的:研究天然牙-种植体共同支持式固定义齿的临床疗效。方法:用常规种植手术方法为10例患者植入Replace select种植体11枚,经愈合期后,采用天然牙-种植体支持的金属烤瓷固定义齿修复缺失牙。采用自觉症状、种植体与天然牙的临床动度、X线片、菌斑指数、牙龈出血指数和牙周袋探诊深度作为观察指标,进行为期2年的临床观察。结果:11枚种植体行使功能良好,所有种植体均无松动及种植体周围X线透射影等种植失败症状,种植体周骨降低的均值为(1.0±0.25)mm,其近、远中邻面骨降低值无明显差异(P>0.05);天然牙-种植体支持的烤瓷冠表面多数位点有不同程度的菌斑堆积,黏膜有炎症的位点占所有检测位点的29.5%;种植体所有检测位点的探诊深度均值为(2.4±0.8)mm,天然牙的检测位点的探诊深度均值为(2.3±0.9)mm。结论:天然牙-种植体共同支持式固定义齿修复2年内的临床疗效满意,是一种临床可采用的义齿修复方法。 Objective: To evaluate the clinical effects of tooth-implant supported fixed dental prosthesis. Methods: 11 Replace-select implants were placed in the mandibles of 10 patients. After healing periods, tooth-implant supported fixed dental prothesis were finished. Patients' self-feeling, tooth-implant's clinical mobility, X-ray, plaque index (PLI), suelus bleeding index (SBI), probing depth (PD) were measured during 2 years follow-up. Results: No signs of mobility or periimplant radiolucencies were observed in any cases. All implants healed well with good functions. The average bone loss adjacent to implants was( 1.0±0.25)mm, and there was no difference in bone lose between the distal and medial aspects of the implants(P〉0.05). The PLI of the fixed restorations were compared. The prevalencies of peri-implant and peri-tooth mu- cosal inflammation,as indicated by SBI of 1.0 or more, was 29.5%. The average PD of the implants was (2.4±0.8)mm. The average PD of the teeth was (2.3±0.9)mm. Conclusion: The fixed restorations supported by tooth-implant combination were proved to be satisfy by the clinical study of 2 years, and it is a good restore way for the missing teeth patients.
